Requirements
  • A Bachelor's degree in Nursing is required.
  • Exceptional communication skills are required.
  • Ability to work effectively in a team structure is required.
  • Ability to interact respectfully with a diverse group of stakeholders across disciplines and educational levels is required.
  • Knowledge and skills in quality improvement are required.
  • Knowledge of medicine, public health, and health services issues are required.
  • Working knowledge of standard desktop computing software packages, including word processing, spreadsheet, presentation, and internet browser software, is required.
Responsibilities
  • Provide strategic oversight of population health programs, including Chronic Care Management, Certified Community Behavioral Health Clinic Care Coordination, and Community Health Worker initiatives.
  • Deliver high-quality, patient-centered care and advance value-based performance.
  • Ensure compliance with regulatory requirements.
  • Drive La Clinica de Familia's movement toward an outcomes-focused, team-based care model focused on improving health equity, quality, and cost.
Desired Qualifications
  • A Master's degree is preferred.
  • A minimum of 2 years of clinical health outcome experience is preferred.
  • Previous management experience is preferred.
  • Familiarity with health care delivery in community health settings is preferred but not required.
