- Provide strategic oversight of population health programs, including Chronic Care Management, Certified Community Behavioral Health Clinic Care Coordination, and Community Health Worker initiatives.
- Deliver high-quality, patient-centered care and advance value-based performance.
- Ensure compliance with regulatory requirements.
- Drive La Clinica de Familia's movement toward an outcomes-focused, team-based care model focused on improving health equity, quality, and cost.
- A Master's degree is preferred.
- A minimum of 2 years of clinical health outcome experience is preferred.
- Previous management experience is preferred.
- Familiarity with health care delivery in community health settings is preferred but not required.
La Clinica de Familia is a community-based nonprofit providing medical, dental, behavioral health and educational services in southern New Mexico. Its programs serve people of different ages and backgrounds, including families who face financial, geographic or cultural barriers to care. Through clinics and community programs, the organization coordinates primary health services, counseling, prevention and family support.
