- An active Registered Nurse license in New Mexico is required.
- At least one year of hospice experience is required.
- Strong clinical judgment and the ability to work independently in the field are required.
- Proficiency with mobile documentation devices such as iPads and iPhones is required.
- Reliable transportation, a valid driver's license, and current auto insurance are required.
- Deliver skilled nursing care in alignment with each patient’s hospice plan of care.
- Perform in-home assessments to evaluate patient symptoms, goals, and eligibility.
- Provide education and emotional support to patients and families about the hospice philosophy and services.
- Collaborate with the interdisciplinary care team to create and adjust individualized care plans.
- Manage pain and symptoms through pharmacological and non-pharmacological methods.
- Communicate with physicians, caregivers, and other providers to ensure quality, coordinated care.
- Support patients and families at end-of-life, including guidance through bereavement resources.
- Document care accurately and in a timely manner using electronic medical record systems.
