Requirements
  • Holds a current Licensed Practical Nurse license in Virginia.
  • Has a minimum of one year of nursing experience.
  • Has current cardiopulmonary resuscitation certification.
  • Can function with minimal supervision.
  • Can manage multiple projects and changing priorities.
  • Can employ critical thinking skills.
Responsibilities
  • Accepts client assignments from the Registered Nurse Case Manager.
  • Provides nursing care through home assessment, nursing diagnoses, and physician-approved care plans.
  • Performs treatments ordered by the physician and delegated by the Registered Nurse Case Manager.
  • Observes signs and symptoms and reports them to the Registered Nurse Case Manager.
  • Notes changes in the physical or emotional status of clients.
  • Promotes client and caregiver independence in self-care.
  • Uses electronic documentation of client care and complies with agency policies regarding timely submission of documentation and data synchronization.
  • Follows agency policies regarding universal precautions and safety guidelines.
  • Fulfills other assignments as requested by the Registered Nurse Case Manager, Vice President of Clinical Services, or Director of Professional Services.
