Responsibilities
  • Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and facilities.
  • Diagnose, manage, and treat chronic and acute conditions with support from clinical operations, the triage team, and medical director leadership.
  • Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
  • Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
  • Complete comprehensive evaluations, including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
  • Use the integrated data ecosystem to track hospitalizations, coordinate post-discharge visits within 72 hours, and drive health outcomes.
  • Collaborate with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
