Requirements
  • Membership in good standing with the applicable regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, or Ontario College of Social Workers and Social Service Workers.
  • Recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Time management, prioritization, and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports, and forms.
  • A valid driver's licence and access to a reliable vehicle.
  • Proficiency in a Windows environment.
  • Advanced oral and written proficiency in English and French is essential.
  • Proof of COVID-19 vaccination status is required before the start date as a condition of employment.
Responsibilities
  • Assess care needs in collaboration with patients and their families, determine eligibility for services, and develop individual care and service plans.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish a helping relationship with patients and their families.
  • Balance patient needs and choices with available resources while respecting patients' values and preferences.
  • Work in the initial Community Long-Stay assignment, with possible assignment changes within the occupation based on operational needs.
Desired Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury, mental health, geriatric, or pediatric populations.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
