Requirements
  • Maintain a current Licensed Practical Nurse license in the state of New Mexico.
  • Have a valid driver’s license, auto insurance, and reliable transportation.
  • Have strong communication and time management skills.
  • Be comfortable working independently in patient homes.
  • Be able to adapt care to a variety of home environments and patient needs.
  • Use mobile electronic medical record systems to document care.
  • Provide compassionate, high-quality patient care.
Responsibilities
  • Deliver skilled nursing care under the supervision of a registered nurse and in accordance with patient care plans.
  • Monitor patient conditions and report changes to the supervising clinician or physician.
  • Perform treatments and administer medications as prescribed.
  • Educate patients and caregivers on managing illness and treatment plans.
  • Document all care activities accurately and promptly using electronic medical record systems.
  • Collaborate with interdisciplinary team members to support patient goals.
  • Travel locally to provide care to patients throughout the Albuquerque area.
  • Participate in team meetings, ongoing education, and quality improvement initiatives.
Desired Qualifications
  • Experience in home health or a similar clinical setting is preferred but not required.
