Requirements
  • An associate degree or diploma in Nursing is required.
  • Three years of clinical experience is required.
  • An active Registered Nurse license in the state of practice (Illinois or Wisconsin) is required.
Responsibilities
  • Collaborate with the patient and family, social workers, physicians, the interdisciplinary team, and payer representatives to support patient progression through the continuum of care efficiently and cost-effectively.
  • Identify, initiate, and manage care coordination and discharge planning to improve patient flow and throughput, continuity of care, safe transitions, patient satisfaction, patient safety, and length-of-stay management.
  • Provide expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs.
  • Monitor cost-effective care to ensure services are delivered in the most clinically appropriate setting.
  • Facilitate compliance with regulatory requirements and organizational performance standards, including demonstration of core competencies.
Desired Qualifications
  • Prior case management experience is preferred.
  • Quality/process improvement experience is preferred.
  • A bachelor's degree in Nursing is preferred.
  • Case Management certification (ACM, CCM, or RN-BC) is preferred.
