Requirements
  • Graduation from an accredited nursing school and a current state license for Licensed Practical Nurse or Registered Nurse are required at all times.
  • New graduates are welcome; one or more years of nursing experience is preferred rather than required.
  • Proficiency in common digital tools and applications, including Microsoft Office programs such as Outlook, Excel, and Word, web-based platforms, electronic health record systems, and applicable human resources systems.
  • Ability to walk or stand for extended periods and move throughout the community, resident apartments, and other community areas.
  • Ability to stoop, kneel, crouch, crawl, climb, and balance as occasionally required by the job.
  • Ability to communicate effectively and identify and interpret written information.
  • Ability to lift up to 50 pounds independently and up to 200 pounds with assistance.
  • Follow all infection prevention and control protocols, including use of personal protective equipment when necessary.
  • Complete required training and ongoing education initiatives and maintain current knowledge and skills.
  • Offers of employment are contingent upon successful completion of a background check and drug screening, required health assessments such as tuberculosis testing or physical evaluations, and use of designated personal protective equipment as required by policy and law.
Responsibilities
  • Provide services in accordance with the nurse practice act and state and federal nursing center regulations.
  • Work with the Director of Nursing to provide direction and oversight to care staff regarding residents’ needs.
  • Review and revise assessments and care plans at admission, periodically as required by regulation, and when a resident’s physical or psychosocial condition changes.
  • Review and transcribe physicians’ orders, provide ordered medical treatments, and assist physicians with resident examinations as needed.
  • Initiate resident education plans according to individualized resident needs, physician direction, and community policy, including instruction for residents and families.
  • Observe residents, communicate condition changes to physicians, health care proxies, and supervisors, and provide timely interventions as instructed.
  • Administer and document medications according to the five rights: right medication, route, resident, time, and dose.
  • Supervise and monitor medications for residents who self-administer when applicable.
  • Document observations, red flags, incidents, interventions, communications, clinical procedures, and daily medication assistance and care throughout the shift.
  • Assist and monitor residents with nutritional and hydration needs and document appropriately in the nursing record.
  • Assist residents with activities of daily living, including bathing, oral care, toileting, and dressing.
  • Assist residents in scheduling medical, dental, and laboratory appointments.
  • Use proactive interventions, monitoring, and safety practices in resident rooms and during rounds.
  • Respond promptly to residents’ call lights.
  • Assist residents with transfers and ambulation using proper body mechanics.
  • Take temperatures, pulses, blood pressures, and other vital signs to detect deviations from normal and assess resident conditions.
  • Maintain a restraint-free environment.
  • Support fall-prevention programs for residents who need them.
  • Respond to life-saving situations according to nursing standards, policies, procedures, and protocols.
  • Respond appropriately to personal calls from other health center units in the community.
  • Provide first aid, call emergency personnel, and call family members at residents’ request as needed.
  • Review resident service plans for assisted living and memory care.
  • Encourage residents to participate in scheduled facility activities and assist them in getting to activities as needed.
  • Assist with post-mortem care as instructed.
  • Participate in oversight and management of the medication delivery system, including order entry, medication receipt and tracking, daily medication administration record documentation, narcotic counts, and timely medication check-in.
  • Notify the pharmacy and supervisor of medication issues and work to resolve them promptly.
  • Promote each resident’s physical, personal, and emotional well-being and support the highest possible level of self-care and independence.
  • Understand and follow residents’ advance directives and wishes.
  • Ensure effective communication between community staff and residents’ families concerning residents’ physical, personal, and emotional well-being.
  • Take direction from management, use initiative, and engage in conflict intervention and resolution for residents, families, employees, and others as needed.
  • Perform other duties as assigned.
  • Participate in staff activities that support teamwork, unity, and morale.
Desired Qualifications

  • Cardiopulmonary resuscitation or Basic Life Support certification.
