Requirements
  • A current, unrestricted registered nurse (RN) or licensed practical nurse/licensed vocational nurse (LPN/LVN) license in the state, or a multistate compact license.
  • At least 2 years of clinical nursing experience; hospice, palliative care, home health, oncology, cardiology, pulmonology, or acute care experience is preferred.

  • Ability to manage a territory and meet activity and referral targets.
  • A valid driver's license, reliable transportation, and current auto insurance.
  • Proficiency with customer relationship management (CRM) systems, electronic medical records (EMR), and Microsoft Office.
Responsibilities
  • Identify, research, and qualify target accounts, including hospitals, oncology, pulmonology, cardiology, primary care, and internal medicine practices.
  • Build and maintain a territory plan that prioritizes accounts by referral potential and patient population.
  • Develop relationships with physicians, nurse practitioners, case managers, discharge planners, social workers, and practice managers.
  • Map each account's decision-makers, referral workflow, and barriers to timely hospice or palliative referrals.
  • Record all visits, contacts, and account activity in the CRM within 24 hours.
  • Present Hospice de la Luz as a value-add partner that supports providers, patients, and families through serious illness.
  • Educate providers and staff on hospice and palliative care eligibility, Medicare hospice benefit basics, and disease-specific indicators for decline.
  • Deliver in-services and lunch-and-learns on goals-of-care conversations, symptom management, and end-stage cardiac, pulmonary, and oncology trajectories.

  • Share relevant quality, outcome, and satisfaction data with referral partners.
  • Serve as the clinical point of contact for referral sources and respond promptly to referral inquiries.
  • Coordinate with case managers, physicians, intake, and admissions to support timely, safe transitions from hospitals or clinics to hospice or palliative care.
  • Meet with patients and families when requested to explain services and support informed decision-making.
  • Follow up with referral sources on referral outcomes and address service concerns promptly.
  • Track referral trends, competitor activity, and market changes, and share insights with leadership.
  • Collaborate with clinical operations, intake, and the interdisciplinary team to keep service promises made to referral partners.
  • Participate in business development meetings, community events, and professional associations.
  • Conduct activities in line with federal and state regulations, including HIPAA, the Anti-Kickback Statute, and CMS hospice Conditions of Participation.
  • Follow policies on marketing, patient choice, and ethical referral practices; do not pressure patients or providers or offer anything of value in exchange for referrals.
Desired Qualifications
  • At least 1 year of healthcare sales, marketing, liaison, or community education experience.
  • Established relationships with hospitals and specialty practices in the territory.
  • Bilingual ability in English and Spanish.

About the company

Jet Health provides home-based medical and hospice care in midsize markets. Clinicians visit patients at home to deliver services for conditions such as congestive heart failure, orthopedic needs, and infusion nursing, including care for catastrophically injured individuals, while coordinating with families and managing treatment plans. Growth comes from organic expansion, acquisitions of other home health and hospice providers, and building new operations, along with partnerships with referral sources, payors, and providers. Its goal is to deliver high-quality, comfortable care at home and to expand its market reach and service offerings through a network of home health and hospice services.
