Requirements
  • Licensed Practical Nurse or Medical Assistant/Nursing Assistant certification, current, active, and unrestricted in the current state of practice and residence within the United States or its territories.
  • Basic computer skills and proficiency with Microsoft Office Suite products, including Word, Excel, and PowerPoint.
  • Good organizational skills and time-management abilities.
  • Excellent verbal and written communication skills.
  • Ability to handle difficult situations tactfully and diplomatically.
  • Effective problem-solving and decision-making skills.
  • Fluent English and Spanish, with strong reading and writing abilities in both languages.
Responsibilities
  • Perform case-management functions under the direct supervision of the RN Case Manager and within the scope of licensure, excluding initial case assessments and formulation of the initial care plan.
  • Review medical data to update case-management plans, including communicating with providers, payers, patients, and primary caregivers.
  • Contact payer offices to understand benefit constraints affecting the plan of action.
  • Contact medical care providers and equipment vendors to verify medical necessity of ordered care or equipment.
  • Arrange care according to patient needs, physician orders, and available benefits.
  • Identify alternative treatment possibilities that may help patients reach wellness goals, and provide payers with a cost-benefit analysis when recommended alternatives lack coverage and could improve the patient's medical condition cost-effectively.
  • Become familiar with community resources and funding sources that can assist patients in obtaining quality health care and conserving health benefit dollars.
  • Document case actions in the computer system for assigned patients, complete case documentation, and prepare timely reports to payers describing actions, results, and continuing case-management plans for review by the Case Manager.
  • Maintain billing in the computer system as appropriate.
  • Maintain contact with providers and patients across the continuum of care to ensure patient needs are met; keep chronic-condition cases open for periodic follow-up on clinical status and additional medical needs.
  • Negotiate with providers to maximize available medical benefits and make network referrals as appropriate.
  • Respond to awareness of non-medical issues involving patient safety or welfare by directing patients or families to appropriate providers or community resources, or notifying appropriate authorities in consultation with the Case Manager.
  • Consult regularly with the Case Manager and keep the Director of Case Management informed about complaints or issues beyond the assistant's competence or expertise.
  • Adhere to company policies and procedures and participate in the Quality Management Program by identifying opportunities for improvement to ensure quality services.
Desired Qualifications
  • Licensed Practical Nursing education.
  • Five years of varied clinical experience or three years of experience in a specialty field.
