Requirements
  • Holds a Master’s degree from a school of social work accredited by the Council on Social Work Education.
  • Holds Licensed Social Worker or Licensed Clinical Social Worker licensure.
  • Has a minimum of one year of experience as a social worker in a hospital, outpatient, nursing home, or home-care setting.
  • Has thorough knowledge of community resources.
  • Has good communication skills.
  • Is able to deal effectively with stressful situations and a fast-paced environment.
Responsibilities
  • Assist in evaluating referrals for admission to the home-health program, including identifying patients who would benefit from services, acting as a resource to hospital social-service departments, nursing personnel, and discharge planners, and investigating patients’ financial situations as appropriate.
  • Assist in identifying patients’ needs and formulating an initial plan of care for patients admitted to the agency’s service.
  • Assess patients, their families, support systems, and home situations, and identify needs or potential needs.
  • Work with nurses and other health-team members to formulate comprehensive plans of care that meet patients’ needs.
  • Perform social determinants of health assessments.
  • Explore community resources and assist patients in qualifying for eligible programs and using available resources to obtain maximum benefit.
  • Follow up Adult Protective Services referrals with letters to Department of Social Services directors.
  • Counsel and support patients and assist them in utilizing or building support systems.
  • Assist patients and their families with the emotional aspects of illnesses and conditions, acceptance of medical conditions, and compliance with medical recommendations.
  • Assist patients in formulating advance directives.
  • Participate in multidisciplinary team meetings, reevaluate goals and plans of care, and offer suggestions to better meet patients’ needs.
  • Assist health-team members with emotional factors related to patients’ health problems and provide resources and suggestions for patient-care planning.
  • Maintain required medical records, including timely documentation of observations, actions, patient progress, and response to care using paper or electronic means.
  • Keep physicians informed of patients’ progress by forwarding appropriate documentation and summaries of care at regular intervals and discussing patients with physicians when appropriate to clarify goals, objectives, and plans of care.
  • Perform other duties as assigned by management.
