Requirements

For the nurse practitioner pathway, graduate from a CCNE-accredited nursing program with a Master's Degree in Nursing.
For the nurse practitioner pathway, hold national certification from a recognized APRN Certification Center.
For the nurse practitioner pathway, be licensed or eligible for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
For the nurse practitioner pathway, hold American Heart Association BLS certification; additional Advanced Life Support training may be required at the department's discretion.
For the physician assistant pathway, hold a Bachelor's or Master's Degree as a Medical/Surgical Physician Assistant, as applicable.
For the physician assistant pathway, graduate from an ARC-PA-approved Physician Assistant training program.
For the physician assistant pathway, hold certification from the National Commission for Certification of Physician Assistants.
For the physician assistant pathway, be licensed or eligible for licensure by the West Virginia Board of Medicine or West Virginia Board of Osteopathic Medicine.
For the physician assistant pathway, hold American Heart Association BLS certification; additional Advanced Life Support training may be required at the department's discretion.
Be able to perform work requiring strength or stamina, including lifting, moving, stooping, reaching, positioning, standing, walking, and carrying patients, materials, or equipment weighing 40 pounds or more.
Work closely with others, work protracted or irregular hours, and work around biohazards, infectious diseases, the deceased, water, and electrical hazards associated with patient-care equipment.
Demonstrate proficiency with computers.

Responsibilities

Provide high-quality, patient-centered care as a member of the interdisciplinary healthcare team, assessing, diagnosing, treating, and managing patients within the scope of practice and specialty.
Deliver evidence-based care, promote positive patient outcomes, maintain professional and regulatory standards, and support the department's and organization's clinical, educational, and operational goals.
Coordinate comprehensive management of hospitalized patients, including involving medical and surgical specialty teams to develop a plan of care and reviewing and participating in directing that plan with the clinical team.
Perform diagnostic and therapeutic procedures and peri-operative services, and coordinate ambulatory management, discharge planning, and follow-up as applicable.
Participate in educating residents and medical and nursing students.
Provide healthcare services consistent with state law and professional licensing or certification requirements and hospital privileges, where applicable.
Manage individualized care for hospitalized patients and their families during inpatient hospitalization and plan disposition for immediate post-discharge ambulatory care.
Complete daily rounds, consultations, and pre-procedure evaluations.
Complete integrated assessments.
In collaboration with hospitalist faculty or under established clinical guidelines and protocols, order additional tests or consultations as appropriate.
Perform diagnostic tests and communicate results according to clinical guidelines and scope of practice.
Consult and communicate with clinic and hospital staff about abnormal findings in patient evaluations.
Perform concise, comprehensive clinical evaluations and document findings in the medical record.
Facilitate patient discharge by coordinating appropriate services and tests needed before discharge for outpatient clinic follow-up, as applicable.
Develop individualized plans of care for hospitalized patients, including teaching, discharge planning, and referrals to other healthcare team members as necessary.
Provide care to patients across the age continuum consistent with education and certification.
Identify unstable or complex hospitalized patient-care situations and facilitate appropriate intervention in the clinic or hospital setting.
Recognize emergency situations and initiate effective emergency care.
Evaluate general medicine patients and coordinate ambulatory follow-up, procedures, and tests as applicable.
Assist with collecting outcome measures as applicable.
Apply critical thinking and diagnostic reasoning to identify, evaluate, and manage the individual needs of patients and families.
Use equipment safely.
Base decisions and actions on relevant scientific principles, established standards of care, and research findings.
Use teaching and learning theory and appropriate strategies to educate nursing staff, hospital staff, students, patients, families, and the community.
Promote a learning environment by responding to appropriate patient and family preferences, priorities, and readiness to learn.
Update professional knowledge throughout the year and complete identified clinical competencies.
Serve as a resource for staff caring for hospitalized patients.
Provide educational programs to professional and paraprofessional personnel in the area of expertise.
Participate in community programs and distribute literature as appropriate to the area of expertise.
Document each patient's plan of care, interventions, and evaluations clearly and concisely in the medical record in accordance with approved hospital and WVUM policies and compliance standards.
Collaborate with leaders and staff to achieve improvements in patient outcomes and care.
Lead development and implementation of changes in cardiology practice that positively affect patient outcomes.
Work collaboratively with medical and nursing staff to facilitate problem-solving and provide professional support.
Participate in and initiate interdisciplinary approaches to patient care, program development, and education; serve as a professional role model.
Promote and practice innovation in the expanded role delivering care to inpatients and families throughout hospitalization and in ambulatory settings.
Participate in and provide leadership for committees and task forces.
Document hours and submit billing reports as appropriate.
Review and interpret cardiology data from patient registries, clinical and financial reports, length-of-stay, access-to-care, and cost-per-case reports, and recommend or implement changes as necessary.
Participate in clinical trials and research studies with the Principal Investigator.
Assist hospitalist faculty, administrators, and directors with collecting outcome measures.
Perform other assigned duties and projects.

Desired Qualifications

Acute care or hospitalist experience is preferred for either the nurse practitioner or physician assistant pathway.
For the nurse practitioner pathway, ACNP or AGACNP credentials are preferred, though FNPs will be considered.
For the physician assistant pathway, master's-level education as a Medical/Surgical Physician Assistant is preferred.

Additional Job Description:

The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of hospitalized patients. The APP coordinates involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient and reviews the plan of care with the other members of the clinical team, participates in directing that care, performs diagnostic/therapeutic procedures, peri-operative services, and coordinates ambulatory management and/or discharge planning/follow-up (as applicable). The APP participates in the education of residents and medical/nursing students.

MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Graduate of a CCNE accredited nursing program with Master’s Degree in Nursing.

1. Manages comprehensive and individualized care for hospitalized patients and their families in the inpatient phase of hospitalization and planning disposition for immediate post discharge in the ambulatory setting.

4. With the collaboration of hospitalist faculty or by following established clinical Guidelines and Protocols, orders additional tests or consults as deemed appropriate.

5. Performs and communicates results of diagnostic tests following established clinical guidelines and scope of practice.

6. Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.

7. Performs succinct comprehensive clinical evaluations and documents findings in medical record.

8. Facilitates patient discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments in the clinic (as applicable).

9. Develops a comprehensive individualized plan of care for hospitalized patients which includes teaching, discharge planning (as applicable) and necessary referral to other members of the health care team as necessary.

10. Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.

11. Identifies unstable or complex hospitalized patient care situations and facilitates appropriate intervention in the clinic and/or hospital setting.

12. Recognizes emergency situations and initiates effective emergency care.
