Requirements
  • A bachelor's degree in an approved Human Services field, such as Social Work, Psychology, or Sociology, and one year of postgraduate work with children.
  • Good communication skills.
  • Ability to maintain client-related goal data, including case notes, outreach, and relationships with community partners.
Responsibilities
  • Coordinate, link, monitor, and assist clients and families on the caseload with identified services and needs.
  • Collaborate with community partners and service team members.
  • Work among and facilitate a service team.
  • Identify and link clients and families to community resources.
  • Maintain knowledge of community resources and how to access them.
  • Complete documentation and medical record requirements.
