Requirements
A bachelor's degree in an approved Human Services field, such as Social Work, Psychology, or Sociology, with one year of postgraduate work with children.
Good communication skills.
Ability to maintain client-related goal data, including case notes, outreach, and relationships with community partners.
Availability to work Monday through Friday from 8:00 a.m. to 4:30 p.m.
Responsibilities
Coordinate, link, monitor, and assist clients and families on the caseload with identified services and needs.
Collaborate with community partners and service team members.
Work among and facilitate a service team.
Identify and link clients and families to community resources.
Maintain knowledge of community resources and how to access them.
Complete documentation and medical record requirements.
