Requirements
  • A Nursing Degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.
  • A current, unencumbered, active license to practice as a Licensed Practical Nurse or Licensed Vocational Nurse in the state.
  • The ability to read, write, speak, and understand English and comprehend technical procedures and policy manuals.
  • The ability to apply fractions, percentages, ratios, and proportions to practical situations.
  • Knowledge and skills necessary to provide age-appropriate care to residents.
  • The ability to make independent decisions when circumstances warrant.
  • The ability to deal tactfully with personnel, residents, family members, visitors, government agencies, and the public.
  • Knowledge of nursing and medical practices and procedures and laws, regulations, and guidelines applicable to nursing care facilities.

Responsibilities
  • Provide direct nursing care to residents and supervise day-to-day nursing assistant activities in accordance with applicable standards, regulations, and facility requirements.
  • Direct nursing assistants' daily functions and ensure nursing personnel comply with facility policies, procedures, job descriptions, and nursing service procedures.
  • Review nursing policies, procedure manuals, and job descriptions and recommend revisions.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Prepare written and oral shift reports and recommendations and coordinate nursing services with other resident services.
  • Participate in quality assurance programs, facility inspections, discharge planning, and government surveys.
  • Admit, transfer, and discharge residents and complete related records, forms, reports, and census documentation.
  • Protect resident health information and facility information-system credentials and report suspected unauthorized access or disclosure.
  • Complete, file, and maintain resident records, charts, nurses' notes, accident and incident reports, transfer forms, diet orders, and other required documentation.
  • Receive telephone orders from physicians, transcribe orders to resident records, and report discrepancies to the Nurse Supervisor.
  • Prepare and administer physician-ordered medications and treatments while verifying resident identity and medication accuracy.
  • Maintain medication, narcotic, supply, and equipment inventories and report discrepancies or shortages.
  • Participate in employee performance evaluations, determine shift staffing needs, and make recommendations regarding staffing, transfers, and dismissals.
  • Develop work assignments, provide leadership to assigned nursing personnel, conduct rounds, and address work-performance or staffing issues.
  • Ensure nursing assistants and nurse aide trainees meet training and supervision requirements.
  • Orient and train employees, participate in in-service and continuing education programs, and assist with clinical supervision of nurse aide trainees.
  • Monitor safety, sanitation, hand hygiene, infection control, personal protective equipment, fire safety, and disaster preparedness procedures.
  • Maintain clean and safe nursing stations, medication rooms, treatment areas, and resident care rooms.
  • Recommend and maintain adequate nursing equipment, medications, medical supplies, and safety materials and ensure equipment is operated safely.
  • Review resident care plans, monitor treatment delivery, identify required revisions, and ensure certified nursing assistants follow care plans.
  • Assist with Minimum Data Set assessments, Resident Assessment Protocols, triggers, and care-plan scheduling and revisions.
  • Provide clinical nursing services including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, range-of-motion exercises, and post-mortem procedures.
  • Monitor residents' conditions, notify physicians and next of kin of accidents or changes, coordinate diagnostic and therapeutic services, and support restorative and rehabilitative programs.
  • Inspect treatment areas, monitor dietary compliance, meet with residents and families, and arrange transportation for discharged residents.
  • Report occupational exposures, hazardous conditions, fraud, and other incidents according to facility procedures.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors. Most recently, Hendrickson served as CFO of U.S. Physical Therapy, overseeing 779 outpatient clinics across 44 states. He previously held CFO roles at Capital Senior Living Corporation, managing 128 communities across 23 states, and Belo Corp. PACS Group has announced that co-founder and chief financial officer Mark Hancock will retire on 30 June 2026. He will continue serving on the company's board of directors as vice chairman. Hancock co-founded PACS in 2013 with Jason Murray, starting with two post-acute care facilities in San Diego. Dr Conway currently serves as CEO of Optum, UnitedHealth Group's health services division with revenues exceeding $200 billion.
