Matrix Medical Network

View

Matrix Medical Network contracts with health plans to assess Medicare Advantage, Medicaid and commercial members in their homes. Its Comprehensive Health Assessment combines physical exams, diagnostic screening, medication review and health education. Clinicians record medical and social needs, then help members connect with primary care and community supports; health plans fund the visits. Scheduling and clinical technology coordinate the nationwide field service, which extends client care into patients' homes rather than replacing their regular doctors. Matrix Medical Network is headquartered in Nashville, Tennessee, where its corporate team supports health-plan and clinical operations.
