Requirements
  • Must be at least 25 years old.
  • Must have a High School Diploma or GED.
  • Must be able to read, write, understand, and follow directions.
  • Must have no substantiated findings on the North Carolina Health Care Abuse Registry.
  • Must meet SBI and DMV requirements of LME/MCO contracts.
  • Must have a valid driver's license.
  • Must have three full years of driving experience.
  • Must have an acceptable level of automobile liability insurance.
  • Must be willing to complete all required training as assigned.
  • Must have access to a device with an internet connection at the time services are rendered.
Responsibilities
  • Assist adults with intellectual and developmental disabilities in a day program setting.
  • Implement each individual's Person-Centered Plan and habilitation plan to address identified goals according to the service definition.
  • Instruct, model, and assist consumers with daily living skills, socialization, leisure or recreational activities, and educational and vocational skills.
  • Receive, read, and become familiar with the agency Employee Handbook and personnel policies and procedures.
  • Document services provided in progress notes on the day the service is provided, including observations, consumer participation and progress, behavior changes, and other pertinent consumer information; documentation must be completed in black ink and without white-out.
  • Adhere to the policy prohibiting taking a supported person to a staff member's private residence or leaving a consumer unattended in a vehicle or public place.
  • Submit required documentation by the deadlines specified by the supervisor.
  • Obtain and maintain all training required by the agency and the individual's needs.
  • Attend mandatory staff meetings and consumer conferences, or notify the supervisor in advance when unable to attend.
  • Receive supervision from a supervisor, qualified professional, or eligible associate professional at least quarterly and as needed.
  • Work only authorized hours unless advance approval is obtained from the supervisor or administrator.
  • Report changes in a person's condition, family situation, or needs to the supervisor.
  • Report allegations of consumer abuse, neglect, or exploitation to the supervisor for investigation and incident reporting to the local Department of Social Services.
  • Report destruction or theft of agency property to the supervisor.
  • Report possible fraud against the consumer or agency to the supervisor for investigation.
  • Perform other duties as required by circumstances.
Desired Qualifications
  • Experience with persons with intellectual and developmental disabilities is preferred but not required.
