About the job
- For the Nurse Practitioner option, hold a master's degree from a program accredited by the National League for Nursing Accrediting Commission or the Commission on Collegiate Nursing Education.
- For the Nurse Practitioner option, hold current certification as a Certified Nurse Practitioner in Adult Health or Family Practice from a nationally recognized certifying body.
- Hold authorization for prescriptive authority.
- For the Nurse Practitioner option, have at least 3 years of clinical nursing experience and at least 1 year of clinical experience as a Certified Nurse Practitioner.
- For the Physician Assistant option, graduate from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant or its predecessor organization.
- For the Physician Assistant option, hold a master's degree in any discipline, or a bachelor's degree with 1 year of Physician Assistant experience.
- For the Physician Assistant option, successfully complete the Physician Assistant National Certifying Exam.
- Hold a current, full, active, and unrestricted license or certification in California, or otherwise meet Veterans Affairs qualification standards for the position.
- Successfully complete the Veterans Affairs credentialing and privileging review and a background investigation cleared through the Office of Personnel Management via the Veterans Affairs Security and Investigations Center before starting work.
- Complete required Veterans Affairs mandatory training and maintain current licensure or certification documentation annually.
- Provide health care commensurate with licensure, clinical privileges, and scope of practice to assigned and unassigned patients presenting for care.
- Ensure that each patient's care plan contains clinically indicated medical recommendations and offer all clinically indicated services to patients assigned to the Patient Aligned Care Team.
- Provide leadership to the Patient Aligned Care Team, including shared delegation of appropriate care and care processes to other team members.
- Review available clinical and performance data with the team and focus on continuous improvement of critical team processes.
- Ensure patients have same-day access to face-to-face and virtual care visits during regular clinic hours.
- Collaborate with Patient Aligned Care Team staff to develop personal health plans incorporating care management and coordination appropriate to patient needs.
- Meet performance benchmarks of more than 85% same-day appointment demand met and more than 95% of encounters closed in the Veterans Affairs electronic health record within 72 hours.
- Outpatient Family Medicine or Internal Medicine experience.
- Three years of Certified Nurse Practitioner clinical experience.
