Requirements
  • An appropriate Master’s degree and licensure as a Nurse Practitioner are required.
  • Licensure in the state of Colorado is required.
  • An unrestricted Drug Enforcement Administration license is required.
Responsibilities
  • Conduct thorough assessments of patients’ medical histories, current treatment plans, and goals for care.
  • Perform and document detailed Medicare Face to Face visits for initial and recertification requirements.
  • Anticipate symptom needs, track the course of illness, and conduct acute visits for new or evolving symptoms or conditions in collaboration with the primary care provider.
  • Apply knowledge of common disease trajectories and prognoses.
  • Work with the Bloom primary care provider and Hospice Registered Nurse to develop symptom management plans, prescribe treatments, follow up on changes, and adjust plans as the patient’s condition changes.
  • Collaborate with physicians, nurses, social workers, spiritual counselors, patients’ primary care providers, and assisted living or memory care staff to coordinate holistic care.
  • Provide education and emotional support to patients and families regarding disease processes, treatment options, and end-of-life care, and make appropriate referrals.
  • Assist patients with advance care planning, including advance directives and hospice enrollment.
  • Facilitate prognosis and goals-of-care discussions with patients and families.
  • Address psychological, social, and spiritual needs and recommend grief counseling and support for anticipatory mourning.
  • Maintain accurate, detailed, and timely documentation of required Medicare Face to Face visits.
  • Participate actively in weekly Interdisciplinary Team Meetings.
  • Respond promptly to text messages, calls, and emails.
  • Drive to facilities and patients’ homes for visits while maintaining a territory of facilities with Hospice Registered Nurse case managers.
