Requirements
  • Possession of a valid Registered Nurse license in the state where nursing practice will occur or a multistate license in the nurse’s state of residence.
  • Obtain Basic Life Support certification through the American Heart Association within six months of hire.
  • Demonstrate knowledge of the rationale for appropriate patient care.
  • Manage and prioritize tasks to meet deadlines and adapt positively to change.
  • Communicate through appropriate channels and use the proper chain of command for patient complaints.
  • Handle emergency situations calmly and effectively.
  • Be computer literate, particularly with Microsoft Office products, and proficient with electronic health record software.
  • Maintain interpersonal relationships with coworkers and other members of the healthcare team and organization.
  • Provide customer service in accordance with the organization’s mission.
  • Maintain patient confidentiality according to organizational policy and procedure and Health Insurance Portability and Accountability Act requirements.
Responsibilities
  • Manage electronic health record tasks outside of medication refills and medication prior authorizations, including telephone encounters, actions, documents, and laboratory or diagnostic order follow-up.
  • Notify patients of test results and follow up on outstanding testing or specialty-care orders to ensure completion.
  • Answer patient questions regarding testing and assist with non-medication prior authorizations, including durable medical equipment and diagnostic imaging and testing.
  • Coordinate care for upcoming patient appointments and care between appointments.
  • Review provider schedules and health records before appointments to optimize visits, including immunization status, preventive services, necessary laboratory testing, recent hospital stays, and external health information.
  • Contact patients as needed to discuss care needs and ensure hospital records and external health information are incorporated into the health record.
  • Develop a working understanding of current Uniform Data System measures and continuously assess, plan, implement, and evaluate patient outcomes.
  • Participate in daily care-team huddles to facilitate communication regarding patients seen in the clinic.
  • Answer triage phone calls and work with the primary nurse to schedule or accommodate patients who need to see a provider.
  • Serve as the primary contact for Mission Control.
  • For the diabetes program, assist the primary care nursing team with medication refills, prior authorizations, durable medical equipment, diabetic shoes, and other items needed for continuous patient care.
Desired Qualifications
  • Two years of nursing experience in an outpatient clinic is preferred.
  • A multistate Registered Nurse license is strongly preferred.
