Requirements
  • Experience working with persons with intellectual or developmental disabilities of varying needs.
  • Ability to work in different group home settings and work a flexible schedule.
  • Must pass the Department of Human Services background check and maintain qualified status.
  • Must not be excluded from working in government health programs by the Office of Inspector General.
  • Ability to successfully meet the company’s physical job demands.
  • If using a personal vehicle for company business, possession of a current valid driver’s license and insurance required by state law.
  • Successful completion of a Medication Administration class with a score of 80% or greater, and ability to regularly pass medications correctly without error.
Responsibilities
  • Assist clients with daily care, medical needs, activities of daily living, and mental health supports.
  • Assist individuals with daily routines, including hygiene, domestic tasks, leisure, grooming, socializing, budgeting, safety, studying, homework completion when applicable, and community participation.
  • Plan and implement recreation, leisure, and community activities.
  • Drive clients to and from appointments and community activities using a company vehicle and accompany them during those activities.
  • Assist individuals in establishing and maintaining community relationships and facilitating friendships.
  • Implement programs in accordance with the Coordinated Service and Support Plan, its Addendum, and the Individual Abuse Prevention Plan.
  • Provide person-centered services through practice, action, and language.
  • Maintain working knowledge of each person’s history, skills, vulnerabilities, habilitation plans, and emergency procedures.
  • Participate in the support team process and assist with developing and implementing goals.
  • Administer and document prescribed medications, treatments, diets, and exams as directed by the service plans.
  • Maintain confidentiality in accordance with HIPAA and company policies and procedures.
  • Prepare meals according to planned menus and complete weekly food ordering as needed or scheduled.
  • Assist with daily housekeeping and household maintenance.
  • Complete house quality improvement assessments and address identified issues.
  • Ensure adequate client supervision and remain on shift until a replacement employee arrives.
  • Complete mandatory training required by the organization or 245D regulations.
  • Read and follow company policies and procedures.
  • Remain awake at all times unless working an assigned sleeping shift.
  • Document program data and daily log information accurately and promptly.
  • Collaborate with the team to complete other assigned or needed duties.
  • Work a full-time float schedule with varying hours, days, and locations and sign up for available shifts.
  • Work a minimum of two weekends per month.
  • Move from a scheduled shift to another location as needed.
  • Communicate with the house Supervisor or Coordinator regarding the work schedule and needs of the home.
Desired Qualifications
  • One to three years of experience working with clients with intellectual or developmental disabilities.
  • Interest and ability to maintain various areas of the home and/or client responsibilities.
  • Proven leadership skills.
