Requirements
  • A bachelor's degree in human services or a related field is required.
  • At least one year of experience working with individuals with Intellectual and Developmental Disabilities is required.
  • At least one year of experience as a Qualified Intellectual Disability Professional with individuals with intellectual and developmental disabilities is required.
  • Demonstrated knowledge of day habilitation services, Supported Employment, OPWDD regulations, and Medicaid billing processes is required.
  • Bilingual English and Spanish speaking ability is required.
  • Ability to escort persons supported in community settings and provide direct care when needed.
  • Willingness to provide on-call support and respond to emergencies as required.
Responsibilities
  • Lead the development and implementation of individualized person-centered plans and Staff Action Plans aligned with each person's Life Plan.
  • Oversee daily operations of the Day Habilitation program, ensuring quality services are delivered, documented, and compliant with OPWDD and Medicaid requirements.
  • Support and mentor Direct Support Professionals and staff through training, coaching, and role modeling.
  • Support the Program Director with program management, staff supervision, and emergency responsiveness during high-demand periods.
  • Promote continuous quality improvement and advocate for the rights, dignity, and independence of persons supported.
  • Develop, monitor, and coordinate Staff Action Plans that reflect meaningful, person-centered outcomes.
  • Advocate for persons supported during Life Plan meetings, ensuring their preferences and voices are represented.
  • Participate in screening, admission, and discharge processes to ensure smooth transitions.
  • Facilitate community integration and participation in socialization, volunteerism, recreation, and personal-growth activities.
  • Provide leadership, mentoring, and coaching to Direct Support Professionals and support staff.
  • Conduct staff training on compliance, person-centered planning, Medicaid billing, and program policies.
  • Lead and facilitate meetings with stakeholders and the person's circle of support.
  • Ensure adequate staffing and skill development in alignment with program needs and regulations.
  • Assist with management of program budgets and responsible resource allocation.
  • Respond to emergencies and provide 24-hour on-call support as needed.
  • Ensure all program activities comply with OPWDD regulations, Medicaid billing requirements, and agency policies.
  • Monitor accurate data collection, documentation, and timely submission of billing and reports.
  • Participate in audits, quality assurance, and quality improvement initiatives.
  • Maintain records and ensure documentation, signatures, and dates are accurate and complete.
  • Implement SUS policies and uphold health, safety, and fire-regulation standards.
  • Continuously evaluate program performance and identify opportunities for enhancement.
  • Promote a culture of feedback and collaboration among staff, persons supported, and stakeholders.
  • Ensure adherence to CARF standards and best practices in day habilitation services.
Desired Qualifications
  • Experience in staff supervision, person-centered planning, and program management.
  • SCIP-R certification.
  • CPR and First Aid certification.
  • Proficiency in basic computer applications, including Microsoft Office, Teams, and Windows.
