  • Registered Nurse, or Licensed Practical Nurse with 2 years of lived experience, or a baccalaureate degree with 2 years of lived experience, or an associate degree with 4 years of lived experience, or Certified Medical Assistant with 4 years of lived experience.
  • A high school diploma is required.
  • Candidates with a bachelor's degree in a related field must have 1 year of relevant experience.
  • Candidates with an associate degree or Certified Peer Recovery Specialist status must have 2 years of relevant experience.
  • Certified Medical Assistants must have 4 years of relevant clinical or lived experience.
  • Four years of relevant clinical or lived experience may be accepted in lieu of a bachelor's degree, associate degree, or relevant certification.
  • The ability to communicate with patients, visitors, and coworkers is required.
Responsibilities
  • Provide ongoing supportive services to patients with substance use disorders.
  • Provide education, service coordination, service linkage, and support within healthcare, social service, and justice systems.
  • Provide ongoing care coordination and case management services.
  • Guide linkage to clinical services, including medication-assisted treatment, behavioral health treatment, and social services.
  • Provide education, coordination, navigation, linkages, and support to individuals with substance use disorder.
  • Foster one-on-one supportive relationships built on trust and respect, and encourage engagement and retention in treatment.
  • Meet patients in emergency department visits, addiction treatment appointments, medical appointments, social service appointments, and therapeutic appointments to provide continuity of care.
  • Identify health and wellness needs, barriers, and strengths, including physical health, medical and addiction treatment, social support, occupational and intellectual strengths and goals, financial needs, and housing needs.
  • Perform outreach by meeting patients in the community and act as an ally and advocate for patient services.
  • Attend Addiction Medicine clinical and programmatic conferences and meetings as required.
