About the job

Requirements
  • An active, unencumbered Registered Nurse license in the state of practice is required.
  • A minimum of 2 years of clinical nursing work experience is required.
  • Competent-level business acuity in Medicare Advantage, value-based or full-capitation economics, and serious illness or palliative care program operations is required.
  • Comprehensive knowledge of palliative and serious illness nursing, validated assessment tools (ESAS-r, FRAIL Scale, and ECOG Performance Status), and the Door 1/Door 2 escalation model is required.
  • Strong clinical judgment and the ability to apply Door 1/Door 2 escalation criteria promptly and appropriately are required.
  • The ability to build rapport and trust with seniors and patients managing serious, advanced illness, and with their caregivers and families is required.
  • Verbal and written communication skills with patients, families, specialists, and physicians are required.
  • The ability to recognize early signs of clinical deterioration and caregiver strain or burnout is required.
  • Skill in cross-functional care coordination across specialists, hospitals, primary care providers, and the hospice partner network is required.
  • The ability to conduct home-based, center-based, and facility-based visits across a defined market service area is required.
  • Proficiency in Microsoft Office Suite products, including Excel, Word, PowerPoint, and Outlook, and competency with electronic health records and care management dashboards are required.
  • The ability and willingness to travel locally up to 75% of the time and flexibility to work evenings, weekends, and/or holidays as needed are required.
  • Spoken and written fluency in English is required.
  • The role requires the use and exercise of independent judgment.
  • Basic Life Support certification from the American Heart Association or American Red Cross is required.
  • A current, valid driver’s license and reliable transportation are required.
  • Willingness to obtain a compact Registered Nurse license is required.
Responsibilities
  • Review Tier 1/2 signals and specialist or primary care provider referrals daily, and complete initial outreach within 48 hours.
  • Review each patient’s chart before first contact, including diagnosis, specialist contacts, recent hospitalizations, medications, and advance directives.
  • Build trust and rapport with newly identified patients and schedule the first home- or center-based assessment visit.
  • Coordinate scheduling of the palliative care provider with specialty care team members.
  • Obtain verbal consent by phone and written consent at the first visit, and document preferred contact methods and caregiver information.
  • Confirm and document specialist contacts, including oncology, cardiology, nephrology, and pulmonology, and obtain releases of information; for oncology patients referred through the CPL partnership, confirm the co-management plan with the oncology practice.
  • Conduct head-to-toe assessments covering vital signs, neurological, cardiovascular, respiratory, gastrointestinal, genitourinary, musculoskeletal, skin or integument, pain, and functional status.
  • Administer and score the Edmonton Symptom Assessment System (ESAS-r) and Palliative Care Performance Scale (PPS) at every visit, the FRAIL Scale at initial assessment and every 90 days or sooner with clinical change, and ECOG Performance Status at every visit.
  • Complete detailed pain, nutrition, and medication reviews, including checking available as-needed medications and confirming that the patient and caregiver know how to use them safely.
  • Review and document goals of care, advance directive status, POLST/DNAR status, and healthcare surrogate or power of attorney designation at every visit; confirm family readiness before transmitting a hospice referral.
  • Determine and adjust visit frequency among face-to-face visits, telehealth, and phone check-ins based on patient Tier assignment, ECOG grade, ESAS score, and program guidelines.
  • Maintain weekly contact at minimum with every Tier 1 patient and contact every Tier 2 patient at least every 3–4 weeks, increasing frequency as clinically required.
  • Assess whether a model-generated signal warrants escalation into the panel or a return to surveillance; a fired signal requires assessment, not automatic enrollment.
  • Recognize and follow the two escalation paths to the co-located Physician/NP: Door 1 for a direct physician-level consult request from a primary care provider, specialist, or National Medical Director; and Door 2 when Serious Illness Care Guide escalation criteria are met, including a Tier 1 symptom unlikely to resolve at nurse level, a controlled-substance or complex prescribing decision, or suspected hospice eligibility.
  • Communicate clinical changes, medication concerns, symptom scores, and goals-of-care updates promptly, accurately, and completely to the Physician/NP and patient’s primary care provider.
  • Practice within licensed scope and applicable state regulations, escalating matters that require physician direction rather than managing them independently.
  • Manage the full course of a hospital admission for an enrolled patient, including same-day detection, in-hospital engagement, discharge coordination, medication reconciliation, and a post-discharge home visit within 48–72 hours.
  • Coordinate care with internal and external specialists to keep goal-concordant care central.
  • Schedule or confirm post-discharge follow-up appointments, specialist visits, home health services, and diagnostic testing, making calls on the patient’s behalf when needed.
  • Partner with Specialty Care Coordinators and the hospice partner network to activate a warm transfer when the clinical trajectory, physician order, or patient wishes indicate hospice is appropriate, attesting to family readiness before transmitting the referral.
  • Support patients and families through serious illness with empathy, clear communication, and consistent follow-through.
  • Identify caregiver strain or burnout and coordinate social work referrals as needed.
  • Document every face-to-face, phone, or telehealth patient contact within 24 hours.
  • Record assessment tool scores, Door 1/Door 2 escalations, medication changes, and care plan updates in the program dashboard and meet program documentation standards.
  • Perform other duties as assigned and modified at the manager’s discretion.
Desired Qualifications
  • Case management, oncology, cardiology, pulmonology, nephrology, home health, or hospice/palliative care experience is strongly preferred.
  • Experience with patients with advanced solid tumors, NYHA Class III–IV heart failure, COPD GOLD 3–4, CKD Stage 4–5, or home oxygen dependency is preferred.
  • Bilingual ability matched to the language needs of the assigned market is preferred.
  • Certification in Hospice and Palliative Nursing (CHPN) or Oncology Nursing (OCN) is preferred.
