- Provide routine and urgent assessments and manage chronic diseases.
- Conduct post-discharge follow-ups, medication reconciliation, and labs and diagnostics.
- Coordinate with facility staff, specialists, families, and powers of attorney.
- Complete same-day documentation and comply with CMS and regulatory requirements.
- Deliver on-site care to assisted living and RCFE residents across Stockton, Modesto, and Amador for three days per week, with two days remote for administrative and documentation tasks.
