Requirements
- Maintain current state licensure.
- Maintain current liability and malpractice insurance, if applicable.
- Hold a current Licensed Practical Nurse or Licensed Vocational Nurse license to practice in each state where patient care is supervised.
- Hold current Basic Cardiac Life Support certification.
- Have a valid driver's license.
- Hold valid and current Cardiopulmonary Resuscitation certification.
- Hold a valid and current Licensed Practical Nurse license in the state of practice.
- Pass criminal background, drug screening, driving record, Office of Inspector General Exclusion List, and Sex Offender Registry checks.
- Read, analyze, and interpret general business practices, policies and procedures, professional journals, technical guidelines, and governmental regulations.
- Write reports, business correspondence, and procedure manuals.
- Present information and respond to questions from managers, clients, customers, and the public.
- Provide accurate and clear guidance and direction to Company staff and employees.
- Identify Company risks and institute policies or procedures to ensure compliance using applicable law and industry data.
- Analyze data from different sources to draw conclusions and select appropriate courses of action based on available facts, constraints, and probable consequences.
- Work with adult and elderly patients in a home-care setting.
- See and hear adequately to respond to auditory and visual requests.
Responsibilities
- Deliver coordinated nursing care for individual patients or assigned groups of patients.
- Manage and supervise related clinical activities during the nurse's tour of duty.
- Implement the Plan of Care for assigned patients.
- Coordinate with the Case Manager on changes to the Plan of Care, care frequency, and referrals to other services.
- Direct the activities of assigned nursing and ancillary personnel using professional and supervisory discretion and independent judgment.
- Provide direct patient care in accordance with state regulations for Licensed Practical Nurses and Licensed Vocational Nurses.
- Assist with implementing changes in the Plan of Care to improve client care.
- Conduct initial evaluation visits when applicable and regularly reevaluate patient needs.
- Provide nursing services, treatments, and diagnostic procedures requiring specialized skills, assuming responsibility for assigned patients.
- Participate in in-service programs and train and teach other nursing personnel as required.
- Teach clients appropriate self-care techniques and promote health through rehabilitative measures.
- Develop and implement treatment plans under physician direction and the Plan of Care.
- Observe and report symptoms, treatment and drug reactions, and changes in patients' physical or emotional conditions.
- Initiate preventive and rehabilitative nursing procedures appropriate for patient care and safety.
- Maintain clinical and progress notes and provide progress reports to physicians.
- Coordinate patient services with other agencies and healthcare providers as appropriate.
- Counsel patients and families regarding nursing and related needs.
- Supervise Home Health Aide, Home Care Aide, and Certified Nursing Assistant services and prepare written care instructions.
- Work independently without the immediate presence of a supervisor.
- Maintain accurate time records and submit required documentation in a timely manner.
- Report observed or suspected child or adult abuse as required by law.
- Assist the State Nurse Administrator, Clinical Supervisor, and Clinical Manager with decision-making and policy formation for the Home Health Program.
- Participate in surveys, studies, and special projects as assigned.
- Address unexpected patient crises with initiative and urgency.
- Follow safety practices related to exposure to body fluids, chemicals, and hazardous equipment.
- Perform other duties as assigned.
