Requirements
  • Graduate of an accredited school of nursing with a Master’s Degree in Nursing and a nurse practitioner certification.
  • Minimum of two years as a registered nurse in a healthcare setting.
  • Current registered nurse license and nurse practitioner license in good standing for the state in which practicing.
  • Must have good communication, comprehension, and interpersonal skills.
  • Must be able to speak, read, write, and understand English.
  • Must have good working computer skills with basic professional computer software, including the Microsoft Office suite and Windows, and the capacity to learn new software systems quickly.
  • Must be self-directed with strong organizational skills and the ability to manage concurrent tasks and assignments.
  • Must complete annual education requirements, including Compliance, Privacy, Security, and HIPAA training.
Responsibilities
  • Conduct physical exams and comprehensive medical and social histories for healthy individuals and individuals with acute illnesses and chronic disease, make independent decisions regarding management and treatment, and document observations appropriately.
  • Order, perform, and interpret laboratory tests for screening and diagnosis.
  • Prescribe and order appropriate diets, medications, and other treatments, including physical therapy.
  • Perform therapeutic or corrective measures, including urgent care.
  • Refer individuals to appropriate specialist nurses, physicians, or other healthcare providers.
  • Provide information, instruction, and counseling on health maintenance, health promotion, social problems, illness prevention, illness management, and medication use.
  • Evaluate the effectiveness of instruction and counseling, provide additional instruction and counseling as needed, and follow up with noncompliant patients.
  • Manage client care by developing treatment and follow-up plans, monitoring progress, determining when referrals, hospital admission, or emergency-room visits are necessary, arranging further care, and determining when illness is resolved.
  • Assess each client’s social and economic factors and tailor care accordingly.
  • Maintain familiarity with community resources and connect clients with appropriate resources.
  • Establish agreements with clients regarding provider and client responsibilities.
  • Maintain productivity sufficient to meet facility goals.
  • Make client home visits and provide care in the home as necessary.
  • Accept after-hours calls and handle after-hours problems on a rotating schedule.
  • Participate in developing health education materials and clinical practice guidelines.
  • Obtain informed consent from clients when appropriate and necessary.
  • Collaborate in policy creation and appropriate problem-solving processes.
  • Provide care with respect and dignity for residents, clients, families, and staff; report complaints and allegations of abuse, misappropriation of funds or resident property, and other compliance concerns to appropriate supervisors.
  • Perform other duties as assigned.
