Requirements
  • An Associate's degree is required at minimum.
  • Current certification from the American Health Information Management Association, American Academy of Professional Coders, or Association of Clinical Documentation Integrity Specialists is required.
  • At least two years of experience as a Clinical Documentation Integrity Specialist is required.
  • Strong knowledge of medical terminology, coding principles, and healthcare documentation standards is required.
  • Understanding of MS-DRG assignment, coding guidelines, and clinical documentation requirements is required.
  • Familiarity with The Joint Commission, CMS regulations, and VHA directives related to health information management is required.
  • Sponsorship is not available.
  • Ability to sit, stand, walk, lift, squat, bend, twist, and reach above the shoulders during the work shift is required.
  • Ability to lift up to 50 pounds from floor to waist is required.
  • Ability to lift up to 20 pounds is required.
  • Ability to carry up to 40 pounds a reasonable distance is required.
  • Ability to push or pull with 30 pounds of force is required.
Responsibilities
  • Review and analyze health records to identify documentation improvement opportunities in inpatient and outpatient settings.
  • Generate and communicate queries to healthcare providers to clarify clinical documentation.
  • Develop the facility's clinical documentation integrity management program encompassing inpatient and outpatient billable and non-billable services.
  • Focus on inpatient cases, including patient treatment files, and outpatient abstracts.
  • Develop standard operating procedures, workflow processes, and templates consistent with current standards.
  • Implement an improvement plan focused on updating problem lists or SNOMED-CT consistent with ICD-10-CM code sets.
  • Participate in reconciliation activities to review discrepancies in code or MS-DRG assignments.
  • Conduct program evaluation and performance improvement activities.
  • Monitor and report on key clinical documentation integrity metrics, including query rates, response rates, and impact measures.
  • Generate and deliver periodic weekly, bi-monthly, and monthly reports as required.
  • Participate in collaborative meetings with healthcare teams and administrators.
Desired Qualifications
  • A Bachelor's degree.
  • Experience with electronic health record systems such as VistA/CPRS.
