About the company

Conduit Health runs a digital platform that makes it easier for homebound patients in New York with Medicaid or MLTC coverage to get Durable Medical Equipment delivered to their homes. It combines telehealth, automated insurance authorization, and direct-to-patient fulfillment in a vertically integrated model. After signup, a licensed nurse practitioner from its partner network conducts a short telehealth visit, writes prescriptions, and the company handles insurance paperwork and monthly home delivery, with no upfront costs to the patient because services are paid through benefits. The service stands out by handling care coordination and logistics end-to-end, reducing the need for doctor visits and complicated claims, especially for incontinence supplies. Its goal is to reduce weeks of administrative headaches for homebound individuals and their families by simplifying access to necessary medical supplies and post-acute care.

New player Conduit Health aims to simplify DME process for home care providers.

New player Conduit Health aims to simplify DME process for home care providers. March 20, 2026 Conduit Health, which recently raised $17 million Series A funds, strives to give home care agencies the "easy button" for securing durable medical equipment for patients discharged to home from the hospital. "We started Conduit Health to make it really easy for patients who have Medicare and Medicaid, and to some degree managed Medicare and managed Medicaid as well, just to get things that insurance pays for," CEO Natan Wise told McKnight's Home Care Daily Pulse in an interview this week. Wise, who launched the company 16 months ago with Rocky Seftel, formerly of Signify Health, a tech-enabled home care firm that is part of CVS Health Corp., saw an opportunity to help simplify the DME procurement process for agencies and patients. He explained: For a hospital bed to be paid for by Medicare, it needs a prescription from a doctor and a letter of medical necessity from a doctor that is written in a specific way. The letter then goes to an insurance company, which has seven to 14 days to review the doctor's note and authorize the bed. Once that authorization decision comes in, a DME supply company that takes the patient's insurance has to fulfill it. "It's a really complicated process," Wise said. "So much so that the world of DME traditionally only exists at discharge. Like the only folks that have really figured this out is a hospital when they're discharging a patient or rehab when they're discharging a patient. Once the patients already in the home, there's just no infrastructure that helps them navigate this." As an answer to the complexity, Wise and Seftel built a vertical process in which agencies and patients can go to the Conduit Health website and order equipment. "There's no more three different folks you need to work with," he said. "You have a guaranteed counterparty. When you place an order with Conduit Health, it will get to your home covered by your insurance without needing to corral any additional stakeholders." Home care agencies - both Medicare and Medicaid, medical and nonmedical - find particular value in Conduit Health because of the ease of securing equipment for patients and clients, according to Wise. "All you need to do is just send us patients' information - first name, last name, date of birth, contact information and, like, you're done," Wise said. "We get them all of that because we engage with them directly." Some home care agencies have used Conduit Health as a selling point. "They will then go to doctors and hospitals and say, 'Discharge your patients to my agency.' We don't charge them a penny. They purely are just partners for us that helps us reach more patients." Now that it has a toehold in DME, Conduit Health, which works with 250 partners, has bigger plans. It wants to leverage its expertise in services that require prescription authorization to other areas such as licensed home care through Medicaid, nonemergency medical transportation and diabetic meals.

Conduit Health raises $17M to simplify insurance-covered medical equipment access

Conduit Health, a New York-based startup streamlining access to insurance-covered home medical equipment, has raised $17 million in a Series A round led by Drive Capital. XYZ Ventures, Twelve Below and Eniac Ventures also participated, bringing total funding to $22 million. The company's platform, CareOS, uses artificial intelligence to automate insurance authorisation workflows across Medicare, Medicaid and managed care plans. It consolidates clinical evaluation, insurance authorisation and supply fulfilment into a single workflow, reducing administrative burden for patients and caregivers. Since launching publicly in late 2024, Conduit has served over 50,000 patients and built a network covering nearly 90 million lives across 100 health plans. The funding will support expansion of its technology infrastructure and partnerships. Long-term, the company aims to extend beyond medical equipment to other home-based services.
