Supports individuals throughout Jefferson County; primary work is in the individual’s home with occasional travel to community locations.

- Be at least 18 years old.
- Have a high school diploma, General Educational Development credential, or equivalent.
- Pass a criminal background check in accordance with Oregon state requirements.
- Have a valid driver's license, automobile insurance, and a reliable vehicle if transportation duties apply.
- Have smartphone access for documentation, communication, and timekeeping.
- Be able to obtain CPR and First Aid certification; training is provided if needed.
- Complete the required annual training of at least 24 hours.
- Be able to assist with physical demands including lifting up to 50 pounds, bending, squatting, standing, walking, driving, and frequent sitting, talking, and hearing.
- Be available for day, evening, and night shifts, including weekends.
- Demonstrate verbal and written communication, flexibility, adaptability, collaboration, attention to detail, and the ability to maintain accurate documentation and make thoughtful real-time safety decisions.
- Build a supportive, person-centered relationship with the individual and deliver care aligned with the Individual Support Plan.
- Promote autonomy and choice by supporting the individual in exercising rights and making decisions about their life.
- Assist with bathing, dressing, toileting, laundry, dishes, vacuuming, tidying, and other daily living and household tasks.
- Collaborate with the individual, family, and support team to enable the best possible outcomes.
- Facilitate community participation by helping the individual plan and attend social, recreational, and appointment-based activities.
- Support budgeting, grocery and other shopping, and meal preparation when needed.
- Administer medications or support medication protocols after receiving appropriate training.
- Identify and report incidents or changes in the individual’s health, behavior, or circumstances, and maintain required documentation in compliance with applicable regulations.
- Provide transportation in a personal vehicle when required for errands, community outings, or appointments.
- Adapt to day, evening, night, and weekend schedule variations to meet the individual’s needs.
- At least 6 months of experience working with the intellectual and developmental disabilities community or 1 year of caregiving or related experience.
- Comfort around animals, if applicable.
Premier Community Supports provides person-centered services for people with intellectual and developmental disabilities in southern Oregon. Direct support professionals assist individuals at home and in the community through in-home care, day activities, positive behavior support, 24-hour residential services and employment programs. Plans are coordinated with individuals, families, guardians and case managers around safety, skills and personal goals. The organization's work centers on increasing independence and community participation through direct, individualized support rather than facility-based medical treatment.
