Requirements
  • A high school diploma or GED is required; equivalent experience in lieu of education may be considered.
  • A minimum of 1–3 years of case management experience is required.
  • Strong verbal and written communication skills are required.

  • Basic computer literacy, including email, online research, and creating basic documents using Microsoft Office Suite, including Excel, Outlook, and Word, is required.
  • A New York State motor vehicle license, a safe driving record, and availability of a personal vehicle for work are required.

Responsibilities
  • Partner with clients eligible for Enhanced Services to ensure they are referred to and connected with needed care and supports.
  • Provide short-term involvement with a caseload of 25–30 clients.
  • Coordinate services across multiple providers to ensure seamless care transitions.
  • Serve as the primary point of contact for clients, families, and service providers.
  • Facilitate access to mental health and substance use treatment, housing, and social services.
  • Advocate for clients’ needs within healthcare and social service systems.
  • Educate clients about their rights and available resources to empower them.
  • Address barriers to care, including transportation and financial constraints.
  • Respond to clients in crisis by providing de-escalation and connecting them with crisis services.
  • Develop and implement safety plans with clients and their support networks.
  • Monitor clients at risk and coordinate emergency interventions as necessary.
  • Track and document client progress using standardized tools and software.
  • Evaluate intervention and service effectiveness and make data-driven recommendations.
  • Maintain accurate and timely records in accordance with regulatory and organizational standards.
  • Participate in multidisciplinary team meetings and contribute insights and updates on client progress.
  • Collaborate with peer support specialists, therapists, and medical professionals to enhance client care.
  • Provide community partners with information about mental health and substance use issues and resources.
  • Build relationships with community organizations and service providers.
  • Conduct outreach to identify individuals in need of services.
  • Represent the organization at community events and stakeholder meetings.
  • Document all client interactions and progress according to East House standards and regulatory requirements.
  • Provide supervisors and stakeholders with regular updates and progress reports regarding client status and goals.
  • Travel to various program locations as needed.
Desired Qualifications
  • An associate’s degree in Human Services, Social Work, or another related field is preferred.
  • Narcan training is preferred.
