  • Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
  • Successful completion of an approved Hospital Pharmacy Residency Program supplemented with three years of recent related hospital pharmacy experience, including one year of pharmacy leadership and staff education experience, or an equivalent combination of education, training and experience.

Responsibilities
  • Provide individualized pharmacotherapy and counsel patients about their medication therapy.
  • Collaborate with physicians and other health care providers to develop, implement and monitor therapeutic plans.
  • Assist in developing drug therapy guidelines for individual disease conditions.
  • Prepare and submit case studies and articles sharing new knowledge for publication in professional journals.
  • Act as a department resource person within the assigned area.
  • Participate in medication use evaluations, quality improvement initiatives and clinical drug trials.
  • Provide mentorship and supervision to pharmacy students, residents and new staff.
  • Ensure prescriptions are validated and dispensed accurately, resolve medication safety concerns and discrepancies with the health care team, and document professional practice activities.
  • Collaborate with Pharmacy Coordinators to identify pharmacy staff learning needs and implement and evaluate educational strategies and programs through teaching, coaching, orienting and training pharmacists.
  • Develop, implement and evaluate educational materials for ongoing rollout of new policies, procedures, skills and competencies, and work with Pharmacy Coordinators to implement changes as required.
  • Identify education priorities that meet staff learning needs, complete learning needs assessments of individuals or groups, determine appropriate training requirements and learning objectives, and provide education and training on standards, clinical guidelines, professional practice resources, practice-based research procedures and education materials.
  • Provide leadership, direction and guidance on pharmacy education, research and related special project initiatives through training, orientation, planning, implementation and evaluation of continuing professional education in collaboration with Pharmacy Coordinators.
  • Arrange, plan and conduct formal training sessions in response to identified employee learning, training or clinical needs; provide resources and materials to clinical areas and external facilities as required; and collaborate with Pharmacy Coordinators on education strategies and innovations based on organizational goals and evidence-based pharmacy practice.
  • Maintain records of staff orientation and instruction.
  • Coordinate new staff orientation for pharmacists and other designated staff with Pharmacy Coordinators, ensuring familiarity with pharmacy practice standards, clinical guidelines, professional practice resources, education materials and modules; communicate and consult with Coordinators to share information and coordinate educational sessions.
  • Review and evaluate literature, consult with other professionals, attend relevant educational programs, maintain knowledge of current developments, contribute to improvement of pharmacy services and promote continuous learning among staff.
  • Participate in and lead quality improvement initiatives and related audits, and promote practices that ensure safe client, patient and resident care and improve service.
  • Participate on clinical committees and working groups as required, providing input and recommendations to the site pharmacy leadership team.
  • Provide clinical pharmacy and drug distribution services in assigned areas to maintain practice skills and proficiency.
  • Perform other related duties as assigned.

$1.3M funding will help more Indigenous elders in Chilliwack with at-home care. Published 8:45 am Saturday, March 14, 2026 An at-home program that offers care to Indigenous elders in the Fraser Valley will be expanding, with the main focus on Chilliwack, thanks to $1.3 million in new funding. The Indigenous Caregiver Support Model (ICSM) is a home-based program that supports elders with "complex needs to age safely at home and in their community," according to a Fraser Health press release. It is one of five programs in B.C. benefitting from a total of $6.3 million in funding from Health Research BC over the next three years. The funding will advance research to help B.C. seniors age comfortably in their own home. The ICSM program was launched in 2023 as a pilot project and has supported 33 elders and their caregivers in the Eastern Fraser Valley, including Chilliwack. Its aim is to reduce caregiver strain and improve elder wellbeing by providing culturally safe, coordinated in-home support, training, and financial assistance for Indigenous families caring for elders. It is co-designed by Fraser Health Indigenous Health, Stó:lō Service Agency, First Nations Health Authority, and Métis Nation British Columbia. "This model is an example of what is possible when care is built on relationships, trust and community," said Amanda LaBoucane, interim vice president, Indigenous Health and Cultural Safety. "Many elders and caregivers in the Chilliwack area have experienced firsthand the impacts of residential schools and the child welfare system. By strengthening caregiver capacity and confidence while bridging into integrated clinical care teams run by and for Indigenous people, we see improved health and wellness and stronger relationships within and between communities." The program is currently operating in the Fraser Valley, including Chilliwack, but the expansion will focus on urban Chilliwack as that is an identified need, said Carrie Stefanson with Fraser Health. Caregivers receive skills training and an honorarium to help reduce the economic strain of caregiving, while elders receive regular home visits from Indigenous-led clinical teams who provide culturally safe, relationship-based care, rooted in trust, kindness and respect, according to a Fraser Health.
