- An active Colorado physical therapist license in good standing is required.
- Strong clinical judgment and the ability to work independently in the field are required.

- Competency in core home health physical therapy scope is required, including transfers, gait training, balance and fall prevention, assistive-device recommendations, strengthening, functional mobility, home safety assessment and modification education, and patient/caregiver coaching with practical home programs.
- Own the patient caseload as a case manager and remain accountable for each patient's therapy plan, progression, and outcomes.
- Complete comprehensive in-home assessments and deliver skilled physical therapy visits.
- Build and execute plans of care covering gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set measurable goals, track objective progress, and adjust treatment when patients do not progress.
- Coordinate with the multidisciplinary care team by communicating changes in condition, aligning on orders, and maintaining care-team progress.
- Manage treatment and outcomes directly, including supervising physical therapist assistants where appropriate.
- Coach patients and caregivers on practical home programs.
- Document promptly and thoroughly in Homecare Homebase.
- Prior home health experience is preferred.
- Experience with Homecare Homebase or a similar home health electronic medical record system is preferred.
- Deeper clinical experience with high-acuity physical therapy populations is preferred.
- Experience supervising or mentoring physical therapist assistants and collaborating on care plans is preferred.
- Strong organization and time-management skills are preferred.
