Requirements
  • An active Colorado physical therapist license in good standing is required.
  • Strong clinical judgment and the ability to work independently in the field are required.
  • The ability to escalate changes in patient condition to a registered nurse or physician and clearly document the rationale is required.
  • Strong communication and documentation skills are required.
  • Reliable follow-through on orders, visit frequencies, and patient goals is required.
  • A current driver's license and the ability to travel daily across the assigned territory are required.
  • The ability to carry and transport basic therapy equipment as needed is required.
  • Competency in core home health physical therapy scope is required, including transfers, gait training, balance and fall prevention, assistive-device recommendations, strengthening, functional mobility, home safety assessment and modification education, and patient/caregiver coaching with practical home programs.
Responsibilities
  • Own the patient caseload as a case manager and remain accountable for each patient's therapy plan, progression, and outcomes.
  • Complete comprehensive in-home assessments and deliver skilled physical therapy visits.
  • Build and execute plans of care covering gait and balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
  • Set measurable goals, track objective progress, and adjust treatment when patients do not progress.
  • Coordinate with the multidisciplinary care team by communicating changes in condition, aligning on orders, and maintaining care-team progress.
  • Manage treatment and outcomes directly, including supervising physical therapist assistants where appropriate.
  • Coach patients and caregivers on practical home programs.
  • Document promptly and thoroughly in Homecare Homebase.
Desired Qualifications
  • Prior home health experience is preferred.
  • Experience with Homecare Homebase or a similar home health electronic medical record system is preferred.
  • Deeper clinical experience with high-acuity physical therapy populations is preferred.
  • Experience supervising or mentoring physical therapist assistants and collaborating on care plans is preferred.
  • Strong organization and time-management skills are preferred.
