Requirements
  • A minimum of one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current cardiopulmonary resuscitation certification is required.
  • Current automobile insurance and a valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice services, hospice disciplines, community resources, general nursing practice, managed care principles, Medicare, Medicaid, Accreditation Commission for Health Care guidelines, and human resource management principles is required.
  • Excellent patient assessment skills, documentation and program management skills, and effective oral and written communication skills are required.
  • The nurse must be able to operate nursing assessment tools, computers, facsimile equipment, a copier, and a cell phone or beeper, and must maintain transportation for work.
  • The nurse must be able to work in patients’ homes and/or institutional settings and automobile environments, with potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
  • Sensitivity to end-of-life issues, tact with patients and families, crisis support ability, flexibility, and effective communication with people of all socioeconomic backgrounds are required.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout the hospice journey.
  • Deliver and document skilled, hands-on nursing care according to each patient’s individualized plan of care in the patient’s home, assisted living facility, or other residence.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments, and provide education to patients, family members, and caregivers.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate with interdisciplinary teams including physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other disciplines.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their loved ones.
  • Teach caregivers about safe care techniques and provide practical and emotional support.
  • Communicate changes in patient condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care providers, and hospice physicians.
  • Educate patients and caregivers about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, hospice philosophy and services, and symptom and pain management.
  • Support home health, palliative, and hospice referrals and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient’s care plan warrants participation.
  • Participate in on-call and patient support activities and conduct on-call services in a clinically competent and responsive manner.
  • Follow infection-control, compliance, regulatory, and policy standards.
  • Support quality improvement and scheduling initiatives.
