Requirements
  • Current Registered Nurse license in the State of Texas.
  • American Heart Association Basic Life Support certification continuously maintained; onboarding with American Heart Association or Red Cross CPR is permitted before transitioning to American Heart Association certification at first renewal.
  • Advanced Specialty Courses completed within the timeframes specified by Attachment A and continuously maintained.
  • National specialty certification attempted within four years of hire for eligible roles and continuously maintained thereafter.
Responsibilities
  • Provide direct nursing care to an assigned patient population, service line, or clinic department within the Texas Nursing Practice Act scope of practice.
  • Collect pertinent data through initial and ongoing focused assessments and conduct comprehensive assessments of patients’ physical, psychosocial, and emotional needs.
  • Use appropriate assessment tools, analyze data, verify diagnoses and issues with healthcare consumers and interprofessional colleagues, and identify expected outcomes.
  • Develop, prioritize, implement, and modify individualized evidence-based plans of care in partnership with patients, families, and the healthcare team.
  • Collaborate with patients, families, registered nurses, licensed vocational nurses, advanced practice nurse practitioners, physician assistants, physicians, medical assistants, nursing assistants, and administrative support staff to address patient needs.
  • Administer medications and therapeutic treatments using appropriate devices according to clinical guidelines and orders.
  • Recognize and intervene in abnormal responses to medication therapy and initiate corrective therapy.
  • Escalate patient-care concerns or changes in patient status until resolution is achieved.
  • Initiate referrals based on assessed patient needs and coordinate follow-up appointments and care plans.
  • Communicate with patients, families, healthcare providers, interprofessional teams, and community-based resources to support safe transitions and continuity of care.
  • Act as a liaison between specialties and subspecialties and coordinate referrals and follow-up appointments between inpatient and outpatient care.
  • Perform phone triage and manage prescription renewals in an ambulatory setting.
  • Identify barriers to learning and develop and implement patient and family education plans.
  • Use health-promotion and teaching methods, including technology, to communicate disease prevention information.
  • Evaluate patient responses to interventions and progress toward expected outcomes, and revise plans as necessary.
  • Document care according to policy and guidelines in the medical record.
  • Delegate nursing care to unlicensed assistive personnel and assume a lead or charge-nurse role when assigned and appropriately trained.
  • Mentor and train new nurses and support their orientation, competence, and integration into their roles.
  • Participate in clinical research, quality improvement, practice-change reviews, shared governance, and unit decision-making.
  • Maintain current clinical knowledge, skills, competencies, and certifications.
  • Ensure equipment functions correctly, report deficits, integrate connected health technologies, and submit documentation supporting chargeable and billable care.
  • Create a safe and healthy workplace and professional practice environment.
  • Float between care areas, departments, subspecialty clinics, and ambulatory clinics as directed and according to scope and competency.
  • Work alternating shifts or flexible hours, including days, nights, weekends, on-call, and callback shifts, as required by staffing and patient-care needs.
  • Travel to satellite outpatient clinics as needed to support patient access and continuity of care.
