Requirements
  • A current Licensed Practical Nurse license in the state of employment is required.
  • At least one year of experience as a Licensed Practical Nurse in an acute care setting is required.
  • Home care experience is desired.
  • The candidate must effectively communicate with patients, families, physicians, and co-workers.
  • The candidate must remain calm, patient, and accommodating while providing care.

  • The candidate must understand nursing best practices and make appropriate nursing judgments.

  • The candidate must prioritize work and complete it in a timely manner.
  • The candidate must follow directions and work as a team member.
  • The candidate must have a valid driver’s license and auto insurance listing them as a driver.
  • The candidate must read, speak, and write English.
  • The candidate must use computers or personal digital assistants.
  • The candidate must be able to drive to assigned patient locations.
  • The candidate must lift up to 50 pounds with or without assistance.
Responsibilities
  • Provide skilled nursing care in patients’ homes as ordered by the attending physician.
  • Perform care by following order sets and goals established by the registered nurse.
  • Observe and monitor patient conditions.
  • Notify the registered nurse case manager of changes in patient condition.
  • Administer medication as prescribed by the physician.
  • Counsel patients and families about disease or injury processes and their management.
  • Incorporate patients and families in developing the plan of care.
  • Teach patients preventative measures and decision-making to help decrease rehospitalizations.
  • Listen to patients and family members.
  • Complete clinical documentation according to agency protocol and Medicare and federal guidelines.
  • Document treatment, assessments, and patient education.
  • Maintain an active Licensed Practical Nurse license in the state of employment.
  • Communicate schedule changes outside the normal schedule to the scheduler.
  • Participate in on-call requirements and case conferences.
  • Attend in-service training and mandatory agency meetings.
  • Maintain a professional appearance as a company representative.
  • Follow agency policies and procedures.
  • Drive to patients’ primary locations for scheduled visits.
  • Attend assigned visit locations daily.
  • Document subsequent visits within 24 hours to meet Medicare guidelines.
  • Complete and submit required documentation within company deadlines.
  • Provide skilled care within the Licensed Practical Nurse scope of practice.
  • Fulfill Licensed Practical Nurse visits in the assigned geographic area according to patient needs and federal and state requirements.
Desired Qualifications
  • Two years of experience as a Licensed Practical Nurse is preferred.
