Requirements
  • A current, unencumbered, active license to practice as a registered nurse in Pennsylvania must be maintained in good standing with the State Board of Nursing.
  • All required continuing education and licensing requirements must be maintained.
  • The candidate must be at least 18 years of age.
  • The candidate must pass a clear Cisive background check covering the previous seven years.
  • Current CPR certification is required; Basic Life Support certification with skills is preferred.
  • The candidate must meet the facility’s general health requirements, including a medical and physical examination and two-step purified protein derivative testing or an equivalent.
  • The candidate must be able to push, pull, move, and/or lift at least 25 pounds to a height of at least five feet and carry or move that weight at least 50 feet.
  • A nursing degree from an accredited college or university is required.
  • Training in rehabilitative and restorative nursing practices is required.
  • Two or more years of experience, preferably in a long-term care facility, is required.
  • The candidate must be able to read, write, speak, and understand English and read technical procedures and policy and procedure manuals.
  • The candidate must be able to apply fractions, percentages, ratios, and proportions to practical situations.
  • The candidate must be able to make independent decisions, solve practical problems, deal tactfully with personnel, residents, families, visitors, government agencies, and the public, and exercise leadership and supervisory ability.
  • The candidate must be able to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures needed to provide quality care.
  • The candidate must demonstrate knowledge and skills necessary to provide age-appropriate care to residents.
  • The candidate must be knowledgeable about nursing and medical practices and procedures and the laws, regulations, and guidelines governing nursing care facilities.

Responsibilities
  • Assist the Director of Nursing Services in directing the day-to-day nursing activities in accordance with applicable rules, regulations, and guidelines governing the long-term care facility.
  • Participate in developing, maintaining, and updating written policies and procedures for the nursing service department.
  • Ensure that reference materials at nurses’ stations are current and recommend materials that support the department’s daily needs.
  • Ensure that the Nursing Service Procedures Manual is current and accurately reflects nursing procedures performed in the facility.
  • Ensure that nursing service personnel comply with the Nursing Service Procedures Manual and their respective job descriptions.
  • Prepare written and oral reports and recommendations for the Director concerning operation of the nursing service department.
  • Review department policies, procedure manuals, job descriptions, and related materials and recommend revisions.
  • Coordinate nursing services with other resident services to maintain continuity of the residents’ overall care.
  • Participate in the facility’s nursing-service quality assurance program and facility surveys conducted by authorized government agencies.
  • Review and update residents’ written discharge plans and assist with the nursing-services portion of discharge planning.
  • Meet with nursing staff and support personnel to plan departmental services, programs, and activities.
  • Admit, transfer, and discharge residents and complete accident and incident reports as required.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Complete medical forms, reports, evaluations, studies, and charting.
  • Attend facility committee meetings, including Infection Control, Policy Advisory, Pharmaceutical, and Quality Assessment committees.
  • Prepare reports for facility committees and provide information to the Quality Assurance and Assessment Committee.
  • Participate in developing agendas for Care Plan Committee meetings and provide discharge plans for review.
  • Implement established committee recommendations as directed by the Director.
  • Ensure adequate numbers of licensed nurses and certified nursing assistants are available for each tour of duty.
  • Develop work assignments and delegate administrative authority, responsibility, and accountability to charge nurses.
  • Ensure nursing assistants are enrolled in or have graduated from approved training programs and that nurse aide trainees work under direct licensed-nurse supervision.
  • Make daily rounds to ensure personnel perform assignments according to acceptable nursing standards and report findings to the Director.
  • Monitor absenteeism and staffing levels, enforce policies and dress codes, and report problem areas.
  • Review personnel complaints and grievances and make appropriate reports to the Director.
  • Maintain effective working relationships among nursing and other facility departments.
  • Create and maintain a warm, calm, and supportive environment throughout the unit and shift.
  • Meet with the shift regularly to identify and correct problem areas and improve services.
  • Review and evaluate the department’s workforce and recommend staffing, dismissals, transfers, and other personnel actions to the Director.
  • Participate in employee performance evaluations and determine shift staffing requirements.
  • Administer departmental disciplinary action fairly and without discrimination.
  • Report occupational exposures, suspected fraud, and information-security violations according to facility policies.
  • Inform nursing personnel of new admissions, prepare rooms, greet newly admitted residents, and escort them to rooms as needed.
  • Make rounds with physicians, encourage timely physician documentation, and report untimely visits to the Director.
  • Consult with residents, families, and physicians when planning care, treatment, and rehabilitation.
  • Notify attending physicians and next of kin of changes in residents’ conditions.
  • Visit residents daily to observe and evaluate physical and emotional status.
  • Ensure direct nursing care is provided by qualified licensed nurses, certified nursing assistants, or nurse aide trainees.
  • Review nurses’ notes and provide direct nursing care as necessary.
  • Start intravenous lines and obtain sputum, urine, and other specimens for ordered laboratory tests.
  • Take temperature, pulse, respiration, and blood-pressure measurements as necessary.
  • Assist the Charge Nurse in monitoring seriously ill residents and authorize restraints when necessary under facility policies.
  • Ensure registered nurses and licensed practical nurses follow procedures for medication and supply administration, storage, and control.
  • Monitor medication passes and treatment schedules and review medication cards for accuracy and completeness.
  • Inspect treatment areas daily and ensure residents unable to call for help are checked frequently.
  • Meet with residents and family members, report problems to the Director, and assist with corrective action.
  • Arrange transportation for discharged residents and notify families and funeral homes of resident deaths when requested.
  • Participate in developing, planning, conducting, and scheduling in-service training classes.
  • Assist the In-Service Director/Educator with annual training programs covering occupational safety, tuberculosis, health-information privacy, abuse prevention, safety, and infection control.
  • Assist with clinical supervision planning for nurse aide trainees and attend continuing education programs.
  • Assist the Safety Officer and Infection Control Coordinator in developing safety standards, identifying blood and body-fluid exposure risks, and maintaining infection-control procedures.
  • Ensure personal protective equipment is available and used correctly and that staff follow safety, hand-hygiene, fire-safety, disaster-preparedness, and hazardous-condition reporting procedures.
  • Ensure nursing areas, resident rooms, and treatment areas are clean, safe, and sanitary.
  • Ensure personnel use safety equipment and supplies such as back braces and mechanical lifts when moving residents.
  • Recommend departmental equipment and supply needs and maintain adequate stocks of medications, medical supplies, and equipment.
  • Assist with preventive maintenance and safe-operation procedures for nursing equipment and ensure only trained personnel operate it.
  • Monitor nursing procedures to promote efficient use of supplies and maintain Material Safety Data Sheets for hazardous chemicals.
  • Participate in written resident assessments and comprehensive care plans, including goals, care approaches, responsible services, and revisions.
  • Ensure personnel follow resident care plans, review nurses’ notes, and verify that treatments are provided according to care plans and residents’ wishes.
  • Assist the Resident Assessment/Care Plan Coordinator with scheduling and revising the Minimum Data Set, including Resident Assessment Protocols and triggers.
  • Monitor economic conditions and recommend nursing-service adjustments to preserve quality care.
  • Report suspected or known fraud involving false billing, cost reports, or kickbacks.

PACS Group has appointed Carey P. Hendrickson as chief financial officer, effective 27 April 2026. Hendrickson brings nearly four decades of financial leadership across healthcare, senior living and media sectors.
