Requirements
  • A Doctor of Pharmacy degree from an accredited institution is required.
  • A license to practice pharmacy in the state of Florida is required.
  • Completion of a PGY1 Pharmacy Residency or three years of relevant clinical experience, particularly in the ambulatory care setting, is required.
  • Effective communication and management skills are required.
  • Proficiency in interpreting and utilizing data for decision-making is required.
  • Experience teaching, guiding, and mentoring professionals is required.
  • Strong project and program management experience is required.
  • Experience in antimicrobial stewardship, medication policy, medication safety, and formulary management within the ambulatory care setting is required.
Responsibilities
  • Establish and expand clinical services to ambulatory clinics and facilities, and implement and support the organization's antimicrobial stewardship program goals and initiatives.
  • Assist the Outpatient Parenteral Antibiotic Therapy team with the appropriate dosing, indication, and administration of antimicrobial therapies.
  • Participate in Agency for Healthcare Research and Quality ambulatory antimicrobial stewardship program initiatives to enhance the quality of healthcare services provided.
  • Provide pharmacy consultation services for providers caring for patients in ambulatory settings across the health system.
  • Advocate for optimal antimicrobial use to improve clinical outcomes, prevent drug resistance, and enhance safety.
  • Promote the safe and appropriate use of vaccines.
  • Develop medication and vaccine protocols and order sets, and facilitate medication and vaccine utilization reviews and performance improvement projects.
  • Provide mentorship and serve as a clinical resource for staff, pharmacy students, pharmacy residents, and other healthcare providers in the area of expertise.
  • Track key performance indicators to quantify the success of stewardship interventions across ambulatory facilities.
  • Track and report antimicrobial usage and cost metrics to clinical systems, clinical program leaders, and other stakeholders in the ambulatory setting.
  • Participate in ambulatory and system-level antimicrobial stewardship research initiatives.
  • Prepare and deliver antimicrobial stewardship-related presentations to stakeholders and learners.
  • Conduct case reviews of adverse events associated with antimicrobial use and review cases of poor outcomes due to antimicrobial mismanagement.
Desired Qualifications
  • Board Certified Infectious Diseases Pharmacist certification.
  • Previous experience working in an integrated healthcare delivery system.
