Requirements
  • Must be at least 21 years old.
  • Must have a high school diploma or GED.
  • Must be able to read, write, understand, and follow directions.
  • Must have no substantiated findings on the North Carolina Health Care Abuse Registry.
  • Must meet State Bureau of Investigation and Department of Motor Vehicles requirements of Local Management Entity/Managed Care Organization contracts.
  • Must have a valid driver's license.
  • Must have three full years of driving experience.
  • Must have an acceptable level of automobile liability insurance.
  • Must be willing to complete all required training as assigned.
  • Must have access to a device with an internet connection when services are rendered.
Responsibilities
  • Work one-on-one with a person with intellectual/developmental disabilities to help them reach individualized goals and become as independent as possible, in a private home or community location.
  • Implement the individual habilitation plan by instructing the person supported in daily living skills, socialization, employment, leisure, and recreational activities.
  • Document the service provided in progress notes, including observations, consumer participation and progress, behavior changes, and other pertinent information; complete documentation on the day of service in black ink without white-out.
  • Obtain guardian initials or signatures on documentation when required to verify hours worked and services provided.
  • Follow company policies regarding transport to private residences and never leave the person supported unattended in a vehicle or public place.
  • Supervise or assist with housekeeping, meal preparation, grooming, bathing, eating, and other applicable activities.
  • Provide transportation to community services and activities when required.
  • Assist the person supported with money management.
  • Encourage the person supported to make appropriate decisions to maximize independent living skills, including making and keeping appointments.
  • Assist with prescribed exercises taught by an appropriate professional.
  • Perform incidental household services essential to the person supported's care at home.
  • Monitor progress notes, charts, and programming materials, and maintain all required records.
  • Submit required documentation by the deadlines specified by the Service Coordinator or Supervisor.
  • Complete and maintain agency-required training, including at least 12 hours annually.
  • Follow company policies and procedures, including emergency-service and medication-administration regulations.
  • Attend mandatory staff meetings and consumer conferences, or provide advance notice when unable to attend.
  • Receive monthly and as-needed on-site supervision from the Service Coordinator or Supervisor.
  • Work under the supervision of a Qualified Professional in accordance with North Carolina regulations.
  • Follow state regulations applicable to working with the assigned person supported.
  • Work only service-ordered hours unless advance approval is obtained from the Administrator.
  • Maintain therapeutic and adaptive equipment.
  • Report changes in the person supported's condition, family situation, or needs to appropriate personnel.
  • Report allegations of abuse, neglect, misappropriation, fraud, or drug diversion to the Service Coordinator or Supervisor.
  • Perform other duties required by the circumstances.
