Requirements
  • Membership in good standing with the applicable regulatory body: College of Nurses of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario College of Social Workers and Social Service Workers, College of Dietitians of Ontario, or College of Audiologists and Speech Language Pathologists of Ontario.
  • At least 2 years of recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Interpersonal, communication, assessment, problem-solving, and decision-making skills.
  • Time management, prioritization, and organizational skills, with the ability to work independently and cooperatively in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports, and forms.
  • A valid driver's licence and access to a reliable vehicle.
  • Proficiency in a Windows environment.
  • Proof of COVID-19 vaccination status before the start date is required.
  • Availability for a six-week, full-time paid orientation for new Part-Time B hires.
  • Availability of at least five shifts in a two-week period, within 8:00–20:00, not exceeding 37.5 hours per week, with at least one Monday or Friday availability day.
  • Availability for one weekend out of three.
  • Availability for five paid holidays per fiscal year, including either Christmas or New Year's Day, with holiday availability rotated yearly.
  • Availability for 44 calendar weeks per year, with no more than three weeks off during July and August.
  • Part-Time B availability must be submitted in writing according to the stated scheduling process and deadlines.
Responsibilities
  • In collaboration with patients and their families, assess care needs, determine service eligibility, and develop individual care and service plans.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish a helping relationship with patients and their families.
  • Balance patient needs and choices with available resources while respecting patients' values and preferences.
  • Travel in the Sarnia-Lambton area as needed to meet and collaborate with patients and their families to develop individual service plans.
Desired Qualifications
  • Experience working with diverse patient groups, including multicultural, homeless, palliative, acquired brain injury, mental health, geriatric, or pediatric populations.
  • Case management experience or recent related community experience.
  • Ability to speak French or another second language.
