About the job
- An active New York State Registered Nurse (RN) license is required.
- CPR/BLS certification is required.
- Strong clinical assessment and critical-thinking skills are required.
- Ability to work independently while maintaining communication with the clinical team is required.

- Conduct comprehensive patient assessments.
- Develop and implement individualized care plans.

- A minimum of 1–2 years of nursing experience is preferred.
- Home care, geriatric, hospice, or related clinical experience is preferred.
- Experience with electronic health records (EHR) is preferred.
- A valid driver's license and reliable transportation are preferred.
