About the job
- Social worker or nurse qualification.
- At least two years of full-time experience in care coordination, discharge planning, or case management in a medical setting within the last four years.
- Current Basic Life Support certification.
- Basic computer skills and working knowledge of Microsoft Office products, including Word, Excel, and Access.
- Establish and maintain procedures to promptly identify sponsors whose family members have special medical and educational needs.
- Perform administrative duties involving the coordination, processing, review, and maintenance of medical records and information about individuals and families assigned to the installation or preparing to serve in overseas and some continental United States assignments.
- Maintain records to ensure service availability for relocating families.
- Process Command Sponsorship requests and documents.
