- A bachelor's degree is preferred, or an equivalent combination of experience and education may be considered.
- Ability to travel to outreach locations, including medical centers, homeless shelters, housing units, outreach events, and other locations across the catchment area, is required.
- Willingness and ability to travel regularly across various locations, including homeless shelters and Veterans Affairs locations, up to 90% of the time, is required.
- Effective written and oral communication skills are required.
- Ability to work flexibly across office and fieldwork locations on a weekly basis is required.
- Provide outreach through distributing materials, delivering informational and educational presentations, and marketing to community members and potential participants.
- Address crowds of varying sizes and deliver program presentations.
- Provide resource and referral services, particularly for U.S. Department of Veterans Affairs services and entitlements, public benefits, and housing resources.
- Execute outreach plan functions and coordinate resources with SSVF staff and other SSVF programs.
- Interface with representatives from referring agencies and partnering service providers to facilitate program services and maintain a steady flow of referrals.
- Assist program participants in accessing community services, including housing, employment, medical, psychological, substance abuse, vocational, food, and clothing services, through referral-network development, referrals, and follow-up.
- Maintain a small caseload of SSVF participants in accordance with SSVF standards.
- Assist with developing Housing Stability Plans and conducting follow-up recertifications with participants.
- Advocate for participants as appropriate and encourage and support self-advocacy.
- Maintain working relationships with colleagues and program managers.
- Document all services in compliance with agency standards.
- Work as a member of the social service team to develop on-site and off-site socialization, educational, risk-reduction, and skill-building activities supporting permanent housing stability.
- Serve as a liaison to community-based services and groups, including civic, social service, government, business, landlord, and nonprofit organizations.
- Participate in individual supervision, team meetings, case conferences, division meetings, and trainings.
- Travel to outreach locations and engage stakeholders in the field, at medical centers, shelters, and other outreach locations up to 90% of the time.
- Attend occasional events outside normal business hours.
- Coordinate referrals for internal and external housing, employment, and legal services.
- Promote the SSVF Housing First model of service delivery, especially for people facing the highest barriers to accessing and maintaining permanent housing.
- Remain current on new regulations, policies, industry trends, and best practices.
- Lived expertise in homelessness or human services is a plus.
- Ability to work outside of normal business hours, including Veterans Day and occasional weekend hours, is a plus.

S:US helps New Yorkers who face homelessness, disability, behavioral health and addiction challenges, poverty, criminal justice involvement, and trauma access a wide range of supports across New York City and Long Island. It coordinates services such as case management, housing and stabilization, behavioral health treatment, recovery supports, rehabilitation, advocacy, and community-based programs tailored to each person’s needs. By linking housing, health, justice, and community resources, S:US delivers integrated, place-based services that address multiple barriers at once rather than focusing on a single issue. The goal is to help people lead purposeful lives, improve neighborhoods, and break the cycle of poverty for current and future generations of New Yorkers.
