  • Graduate of a Nurse Practitioner program with a Master's degree.
  • Hold a current Delaware Advanced Practice Registered Nurse license or be able to obtain one.
  • Maintain current Basic Life Support certification.
  • Maintain Advanced Cardiac Life Support certification.
  • Maintain National Institutes of Health certification.
Responsibilities
  • Provide clinical care and leadership for the Neurology Service line within an interdisciplinary model of care, with specific attention to patients at high risk of poor outcomes and transitions in care.
  • Perform complete histories and physical examinations and document them in the electronic medical record.
  • Order and collect data using appropriate assessment techniques, supporting diagnostic information, and diagnostic procedures when indicated.
  • Serve as a first responder for nursing and house staff during patient status changes and situations requiring procedural or resuscitative interventions.
  • Evaluate and re-evaluate acutely and chronically ill patients and routinely modify their plans of care as needed.
  • Collaborate with the interdisciplinary health care team to diagnose acute and chronic conditions.
  • Manage diagnostic tests through ordering, interpretation, performance, and supervision.
  • Formulate prioritized differential diagnoses, diagnose complications, and order appropriate interventions.
  • Formulate plans of care, identify expected outcomes, and document care for complex acute and chronic health care needs.
  • Implement and modify individualized plans of care using evidence-based practice guidelines and protocols across the continuum of care.
  • Document patient progress through daily progress notes, transfer and discharge summaries, histories and physicals, plans of care, problem lists, procedure notes, and acute event notes.
  • Maintain timely documentation that meets acute care compliance standards and accurately reflects patient acuity.
  • Communicate and collaborate with the interdisciplinary health care team to promote continuity of care.
  • Work with the Stroke Coordinator to reinforce education for medical and nursing staff.
  • Serve on the stroke committee and attend meetings.
  • Facilitate communication with patients, families, and staff to promote continuity of care.
  • Schedule follow-up appointments with a neurologist before patient discharge.
  • Precept and mentor graduate students and provide education and instruction.
  • Participate in specialty-specific meetings and engage in process improvement.
  • Develop and review current protocols with hospitalists and the Stroke Coordinator.
  • Pursue active engagement in research and analysis of evidence-based practice.
  • Maintain membership in a professional organization and continuing education units.
