Requirements
- At least 1 year of experience as an MDS Coordinator is required.
- Registered Nurse qualification is stated in the subtitle and job description.
- Ability to work in person in a long-term-care, nursing-home, or rehabilitation-center setting.
- Availability for an 8-hour shift, Monday through Friday, holidays, and weekends as needed.
- Ability to commute to or relocate to the IE area.
Responsibilities
- Complete and submit timely and accurate Minimum Data Set assessments.
- Coordinate and participate in care planning meetings.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Monitor and evaluate the effectiveness of care plans.
- Communicate with residents, families, and healthcare providers to ensure that care needs are being met.
- Ensure compliance with federal and state regulations related to Minimum Data Set assessments and care planning.
- Participate in quality improvement initiatives.
