Requirements
  • At least 1 year of experience as an MDS Coordinator is required.
  • Registered Nurse qualification is stated in the subtitle and job description.
  • Ability to work in person in a long-term-care, nursing-home, or rehabilitation-center setting.
  • Availability for an 8-hour shift, Monday through Friday, holidays, and weekends as needed.
  • Ability to commute to or relocate to the IE area.
Responsibilities
  • Complete and submit timely and accurate Minimum Data Set assessments.
  • Coordinate and participate in care planning meetings.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Monitor and evaluate the effectiveness of care plans.
  • Communicate with residents, families, and healthcare providers to ensure that care needs are being met.
  • Ensure compliance with federal and state regulations related to Minimum Data Set assessments and care planning.
  • Participate in quality improvement initiatives.
