Requirements
  • A bachelor's degree in Criminal Justice, Healthcare Administration, Social Work, Nursing, Public Health, or a related field.
  • Demonstrated experience leading investigation teams in a healthcare or home- and community-based services provider environment.
  • Excellent investigation, interviewing, analysis, and professional writing skills, with strong attention to detail and objectivity.
  • Knowledge of incident reporting standards, abuse, neglect, and exploitation prevention, and applicable state regulatory requirements, with working knowledge of the Health Insurance Portability and Accountability Act and confidentiality.
  • Proficiency with incident and case-management systems and analytics tools.
  • Ability to manage multiple priorities under deadlines, exercise sound judgment, and maintain strict confidentiality.
  • Strong interpersonal skills with the ability to influence and collaborate across Operations, Human Resources, Clinical, Legal, and executive leadership.
  • Willingness to travel for high-risk investigations, training, and site support as needed.
  • Completion of a criminal background check, registry checks, drug testing, education verification, and employment history or reference verification as applicable.
Responsibilities
  • Lead the enterprise incident-management lifecycle from intake and triage through investigation, corrective actions, and closure in alignment with regulations, waiver and payor requirements, and company policy.
  • Maintain and update policies and standard operating procedures for incident reporting, abuse, neglect, and exploitation prevention, evidence handling, documentation standards, and record retention.
  • Define risk tiers, escalation thresholds, and severity scoring.
  • Establish metrics and dashboards for timeliness, quality, and outcomes.
  • Lead an Incident Review Committee or Panel and ensure quorum, documentation, and timely follow-through.
  • Maintain an enterprise regulatory reference for reportable events, required notifications, and timelines.
  • Align the incident-management program with Just Culture principles and integrate it with the enterprise risk register.
  • Review internal hotline and local abuse, neglect, and exploitation investigations and provide written feedback and coaching to local investigation and leadership teams.
  • Lead selected high-risk investigations, including serious incidents, complex abuse, neglect, and exploitation cases, and certain unexpected deaths.
  • Ensure investigators follow established procedures, monitor report quality, and conduct regular quality-assurance reviews.
  • Coordinate with Operations, Human Resources, Clinical, and Legal to conduct impartial interviews and manage confidentiality and privilege.
  • Establish conflict-screening and recusal protocols to preserve neutrality.
  • Standardize evidence collection, chain of custody, and scene-preservation guidance.
  • Monitor external reporting and engagement with Adult Protective Services, law enforcement, ombudsmen, and survey agencies when required.
  • Partner with Privacy on protected health information events and with Information Security on cybersecurity incidents affecting care or safety.
  • Develop and deliver training on incident intake, interviewing, documentation, and reporting protocols.
  • Provide professional development, job aids, and templates covering investigation technique, report quality, root-cause analysis, corrective and preventive action writing, and Just Culture coaching.
  • Partner with Learning and Development to embed role-based training and refreshers.
  • Provide training and professional development for local incident-management committees, including root-cause analysis and post-incident debriefs.
  • Coach incident and investigation writers on clear, objective, evidence-based report-writing standards.
  • Own the incident-tracking and case-management system, including configuration, workflows, permissions, and data integrity.
  • Ensure accurate and timely data entry and perform routine audits and reconciliations.
  • Use analytics to identify trends, hotspots, systemic issues, and lessons learned, and prepare regular reporting for executive leadership, quality and risk committees, and external stakeholders as required.
  • Lead post-incident reviews and debriefs, track corrective and preventive actions to completion, and evaluate their effectiveness.
  • Define a common taxonomy and coding system for incident types, precursors, contributing factors, and near misses.
  • Integrate the incident-management system with Human Resources Information Systems and electronic health records where appropriate.
  • Monitor leading and lagging indicators and issue safety alerts and lessons learned across regions.
  • Ensure incident reporting and notifications meet federal, state, local, and payor or waiver requirements and timelines.
  • Serve as a point of contact for external agencies and coordinate document production and follow-up actions.
  • Support readiness for audits, surveys, and inspections, and address findings with action plans and monitoring.
  • Ensure required notifications to guardians or legally authorized representatives and payors are made within mandated timeframes.
  • Manage litigation holds and evidence preservation with Legal and insurers or third-party administrators, and support claims documentation.
  • Coordinate with Safety and Facilities on environmental and Occupational Safety and Health Administration-related incidents and corrective actions.
  • Partner with Regional and Operations leaders to mitigate identified risks and prioritize fixes that improve client safety and service quality.
  • Drive cross-functional initiatives based on trend data and root-cause analyses, including policy changes, staffing and process redesign, and environment-of-care improvements.
  • Promote a culture of safety, transparency, and learning across all service lines.
  • Lead after-action reviews for critical events and incorporate improvements into policy, training, and the environment of care.
  • Sponsor performance-improvement projects tied to incident trends, including falls, medication errors, elopements, and transportation events.
  • Lead, coach, and develop an enterprise investigations team and oversee regional and local investigators and external resources as needed.

  • Multi-state provider experience in intellectual and developmental disabilities or behavioral health, home- and community-based services waiver programs, residential or day programs, and large healthcare facilities.
  • Experience with sentinel or serious incident review.

Additional mental health support for Downtown and beyond. August 12, 2026 Asheville is expanding access to mental health resources for people experiencing crisis, with new partnerships touching both public safety and public transit. The Asheville Police Department and Asheville Fire Department's Resource and Engagement Support Team (REST) have partnered with RHA Health Services, supported by a Vaya Health grant, to place certified mental health professionals alongside first responders on select crisis calls. The pilot runs 60 days. Officers and REST firefighters will have a non-uniformed mental health professional on scene to help de-escalate situations and connect people directly with care, rather than simply making a referral. Learn more Sixty days is a short runway to judge impact, so Asheville Downtown is watching how the pilot performs and whether it becomes a permanent fixture. If it works as intended, it should mean fewer unresolved crisis calls Downtown and a stronger safety net for the people who rely on these services most. Asheville Rides Transit is adding mental health support too. As part of the City's new four-year, $54.9 million contract with RATP Dev, a crisis counselor will be stationed at the ART Transit Center at 49 Coxe Ave to connect riders in need with community resources. This is particularly critical as the transit center remains one of its most challenging areas in terms of calls for services related to safety and cleanliness. Anyone visiting, living, and working Downtown likely regularly encounters people in crisis on the street, and until now the options have been limited. Having trained mental health professionals embedded in those responses means faster, more appropriate care for the person in crisis.
