Requirements
  • Have employment eligibility verification.
  • Have or be able and willing to obtain CPR/First Aid training.
  • Successfully complete a tuberculosis test, background screening, and drug test as a condition of hire.
  • Have reliable transportation, a valid driver's license, proof of insurance and ownership for personal vehicles used during work duties, and the ability to qualify for PATH's insurance coverage.
  • Have two years of experience in healthcare, case management, homeless services, working with vulnerable populations, or a related field. This is preferred rather than required.
  • Gather and interpret data, identify trends, patterns, and insights, and consistently produce accurate, precise work.
  • Work effectively with colleagues, participants, and stakeholders while contributing to team goals.
  • Convey information clearly and effectively, verbally and in writing, to individuals and groups.
  • Take initiative, make decisions proactively, and generate solutions to problems or challenges.
  • Analyze and evaluate information, identify key issues, consider alternatives, and make logical decisions.
  • Maintain ethical and professional standards, integrity, and appropriate professional boundaries.
Responsibilities
  • Conduct intake and individualized needs assessments for participants and collaborate to develop Individualized Service Plans addressing housing goals, income, medical, mental health, substance use, financial, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help participants achieve their Individualized Service Plan goals.
  • Complete program-specific assessments, including the VI-SPDAT.
  • Evaluate strengths and challenges related to short-term and long-term goals and conduct a 90-day review with each participant after initial intake.
  • Maintain documentation standards, complete case file notes in a timely manner, and keep participant files complete, accurate, and current.
  • Maintain confidentiality of participant files in accordance with HIPAA and applicable guidelines.
  • Develop therapeutic and professional relationships with clients and systems or partners to support engagement and housing goals.
  • Provide crisis intervention and risk assessment services focused on safety, problem-solving, coping skills, independent care coordination, and support for individuals with chronic trauma.
  • Use motivational interviewing and trauma-informed care in service delivery.
  • Maintain current knowledge of community resources and use them to provide comprehensive wraparound services.
  • Attend and actively participate in required training sessions and apply learned skills to daily responsibilities.
  • Attend and actively participate in required staff meetings and on-site or off-site events when applicable.
  • Demonstrate commitment to diversity, equity, and inclusion through inclusive service delivery, continued learning, modeling inclusive behaviors, and proactively mitigating bias.
  • Perform other duties as assigned or required.
