  • A current New York State license and registration to practice as a Speech Language Pathologist are required.
  • At least one year of post-licensure rehabilitation experience with adult speech and language impairment populations is required.
  • Good computer skills and experience with electronic medical record systems are required.
Responsibilities
  • Assess, plan, develop, and provide patients or responsible caregivers with a home health speech language pathology plan of care.
  • Identify patient needs for other disciplines and services.
  • Coordinate and evaluate the plan of care with all other disciplines and physicians for an assigned caseload.
  • Instruct patients or responsible caregivers on the projected care plan and expected outcomes as directed by the physician’s treatment plan.
  • Document visits in the electronic medical record system.
