Requirements
  • Graduate of an accredited practical or vocational nursing program.
  • Hold a current, unrestricted Licensed Practical Nurse license in New Mexico.
  • Have at least one year of clinical nursing experience.
  • Hold current CPR/BLS certification.
  • Demonstrate clinical judgment, time management, and communication skills.
  • Be able to work independently in the field while staying connected to the care team.
  • Have emotional steadiness, compassion, and reliability in difficult situations.
  • Have a valid driver's license, reliable transportation, and current auto insurance.
Responsibilities
  • Provide skilled hospice nursing care under the direction of a Registered Nurse and the established plan of care.
  • Administer medications and treatments according to physician orders and agency policies.
  • Monitor and document changes in patients' physical, emotional, and psychosocial status, and promptly report significant findings.
  • Educate patients and caregivers about comfort measures, medications, and what to expect during hospice care.
  • Collaborate with Registered Nurses, social workers, aides, chaplains, and other team members to coordinate patient-centered care.
  • Maintain accurate and timely clinical documentation in the electronic medical record in compliance with CareM standards.
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement activities.
  • Navigate complex family dynamics professionally, empathetically, and with healthy boundaries.

  • Home health, hospice, or acute-care clinical nursing experience.
