About the job
- A Master's or Doctoral Degree from an accredited school of Social Work.
- Active social work licensure (LMSW, LICSW, or LCSW) in the state of practice.
- Minimum of one year of experience as a Master of Social Work in a healthcare setting.
- Working knowledge of community resources and effective interpersonal skills.
- Proof of current CPR certification.
- Reliable transportation, a valid driver's license, and auto liability insurance.
- Conduct ongoing clinical assessments of hospice patients to identify psychosocial, financial, environmental, and community resource needs.
- Develop and evaluate the plan of care in collaboration with the patient, client, representative, if any, and caregiver(s).
- Provide counseling and education to patients, clients, and caregivers as ordered by the physician and indicated in the plan of care.
- Maintain accurate and timely documentation of clinical assessments, interventions, and communication with other healthcare practitioners.
- Use interpersonal relations and communication skills to facilitate positive outcomes for patients and their families.
- Collaborate with the interdisciplinary team to promote coordination of patient care and ensure comprehensive support.
- Participate in the Agency's Quality Assessment and Performance Improvement program and in-service training.
- Stay updated on public and private eligibility standards and requirements while maintaining professional growth and competency.
- Promote the Agency's philosophy and administrative policies to support the delivery of quality care.
